Vincristine Sulfate falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Sage interacts with.

Theoretically, sage might increase the levels and clinical effects of drugs metabolized by CYP3A4.
In vitro evidence suggests that aqueous extracts of sage can inhibit CYP3A4. So far, this interaction has not been reported in humans.

Vincristine is a chemotherapy drug your liver clears using an enzyme called CYP3A4. Sage, at least in lab and test-tube studies, seems to slow that same enzyme down a bit. In theory, if sage blocks CYP3A4, vincristine could clear more slowly and build up in your body, which might mean stronger effects and more side effects. With vincristine, that can show up as nerve-related problems like numbness, tingling, weakness, constipation, or muscle pain, plus a bigger hit to your blood counts.

Here is the reassuring part: this has only been seen in a lab, never reported in real patients, so it stays theoretical for now. Still, with a potent cancer drug like this, I would not add concentrated sage extracts or oil without first checking with your oncologist or pharmacist. Normal cooking amounts are unlikely to matter.

Theoretically, sage might increase levels of drugs transported by P-glycoprotein.
In vitro research suggests that common sage (Salvia officinalis) can inhibit the multi-drug transporter protein, P-glycoprotein. This effect has not been reported in humans.

Here's the gist with sage and vincristine. Your body uses a kind of cellular bouncer called P-glycoprotein to pump certain drugs back out of cells. In lab dish studies, sage appears to slow that pump down. Vincristine relies on P-glycoprotein, so if the pump works less efficiently, more of the drug could linger in your tissues than expected.

Vincristine already carries a real risk of nerve-related side effects like tingling, numbness, weakness, or constipation, so anything that nudges its levels up is worth taking seriously. That said, this is only test-tube evidence and hasn't been seen in actual people, so don't panic. A pinch of sage in cooking is very unlikely to matter. Concentrated sage supplements or oil are a different story, so please clear those with your oncology team first.

Sage is a common kitchen herb that is generally safe in food amounts and is traditionally used for sore throats, digestion, sweating, and memory. Some early research is encouraging for sore throat and menopausal hot flashes, but most uses are not well proven. Concentrated extracts and the essential oil should be used carefully because high doses contain a compound called thujone, which can be harmful.
