Why this applies to Vismodegib

Vismodegib falls under P-GLYCOPROTEIN SUBSTRATES — the group Sage interacts with.

Theoretically, sage might increase levels of drugs transported by P-glycoprotein.
In vitro research suggests that common sage (Salvia officinalis) can inhibit the multi-drug transporter protein, P-glycoprotein. This effect has not been reported in humans.

Here's the gist for vismodegib (Erivedge). This medicine is moved around your body partly by a transporter called P-glycoprotein, which acts like a gatekeeper. Lab studies suggest sage may slow that gatekeeper down. If that happened in people, a bit more vismodegib could stay in your system, which in theory could mean more of its typical side effects, like muscle cramps and spasms, hair thinning, loss of taste, nausea, or fatigue.

The important part: this is only seen in test-tube research, not in actual patients, so it's mostly theoretical. Cooking with sage in food is very unlikely to matter. Concentrated sage extracts or sage oil are a bigger question mark. Since vismodegib is a serious cancer drug, please run any sage supplement past your oncology team or pharmacist first.

Sage is a common kitchen herb that is generally safe in food amounts and is traditionally used for sore throats, digestion, sweating, and memory. Some early research is encouraging for sore throat and menopausal hot flashes, but most uses are not well proven. Concentrated extracts and the essential oil should be used carefully because high doses contain a compound called thujone, which can be harmful.

Erivedge (vismodegib) is used to treat adults with basal cell carcinoma — the most common type of skin cancer — in two specific situations: when the cancer has spread to other parts of the body (metastatic basal cell carcinoma), or when it is locally advanced and has returned after surgery, or when surgery and radiation are not options.

It is not approved for use in children.
