There's preliminary evidence that limonene might be a substrate for cytochrome P450 2C19 (CYP2C19). So far, this interaction has not been reported in humans. However, watch for an increase in the limonene levels when it is taken with drugs that inhibit CYP2C19. Some drugs that inhibit CYP2C19 include cimetidine (Tagamet), fluvoxamine (Luvox), omeprazole (Prilosec); ticlopidine (Ticlid), topiramate (Topamax), and others.

Here's the key thing to understand about this pairing: the concern runs in the opposite direction from what you might expect. The interaction on file is about limonene being broken down by an enzyme called CYP2C19. Vismodegib (Erivedge) is one of the drugs that can block that same enzyme. So in theory, taking them together could slow how your body clears limonene, letting limonene levels creep up, rather than changing your vismodegib.

I want to be honest about how thin this evidence is. It comes only from lab and animal studies, and it has never actually been reported in people. Everyday citrus in food is very unlikely to matter. However, watch for side effects in patients taking limonene and CYP2C9 inhibitors. Some CYP2C9 inhibitors include amiodarone (Cordarone), fluconazole (Diflucan), lovastatin (Mevacor), paroxetine (Paxil), zafirlukast (Accolate), and many others.

Erivedge (vismodegib) is used to treat adults with basal cell carcinoma — the most common type of skin cancer — in two specific situations: when the cancer has spread to other parts of the body (metastatic basal cell carcinoma), or when it is locally advanced and has returned after surgery, or when surgery and radiation are not options.

It is not approved for use in children.
