Telmisartan falls under CYTOCHROME P450 2C19 (CYP2C19) INHIBITORS — the group Limonene interacts with.

There's preliminary evidence that limonene might be a substrate for cytochrome P450 2C19 (CYP2C19). So far, this interaction has not been reported in humans. However, watch for an increase in the limonene levels when it is taken with drugs that inhibit CYP2C19. Some drugs that inhibit CYP2C19 include cimetidine (Tagamet), fluvoxamine (Luvox), omeprazole (Prilosec); ticlopidine (Ticlid), topiramate (Topamax), and others.

Here's the key thing to understand: this interaction is really about what happens to the limonene, not your telmisartan. The concern on file is that limonene gets processed by a liver enzyme called CYP2C19, and drugs that block that enzyme can let limonene build up. Telmisartan, though, isn't a meaningful CYP2C19 blocker. It's cleared mostly through the gut and liver in other ways, so it doesn't really push limonene levels up.

On top of that, this is very early evidence, just lab and animal work, with nothing reported in actual people. Food amounts of citrus are no worry at all. If you're using a concentrated limonene supplement, it's still smart to run it by your pharmacist, mostly to keep an eye on your blood pressure overall.

What Is Telmisartan Used For?

Brand name: Micardis

Telmisartan tablets — sold as Micardis and the generic telmisartan — treat high blood pressure (hypertension). Bringing blood pressure down lowers the risk of strokes and heart attacks.

Micardis and telmisartan tablets are also approved to reduce the risk of heart attack, stroke, or death from cardiovascular causes in patients 55 or older who are at high cardiovascular risk and cannot take ACE inhibitors. This use is for patients with a history of conditions such as coronary artery disease, peripheral arterial disease, stroke, TIA (mini-stroke), or high-risk diabetes with signs of organ damage.
