Telmisartan falls under ANTIHYPERTENSIVE DRUGS — the group Adrafinil interacts with.

Theoretically, adrafinil might reduce the effects of antihypertensive drugs.
Adrafinil is a precursor to the stimulant prescription drug modafinil. Modafinil has been shown to increase diastolic and systolic blood pressure. Retrospective analyses show that patients taking modafinil had a small but increased rate of requiring new antihypertensive drugs or increased use of antihypertensive drugs when compared with placebo.

Here's the simple version: when your body turns adrafinil into modafinil, that modafinil can nudge your blood pressure upward (both the top and bottom numbers). Telmisartan (Micardis) is working to bring your blood pressure down, so the two can end up tugging in opposite directions. In theory, that could make your telmisartan feel a little less effective, and you might see higher readings on your home monitor.

I want to be honest about the strength of this: it's a theoretical concern based on how these drugs work, not something proven to happen in most people. Still, since adrafinil is unregulated and hard on the liver, please loop in your doctor or pharmacist before combining them, and keep an eye on your blood pressure if you do.

What Is Telmisartan Used For?

Brand name: Micardis

Telmisartan tablets — sold as Micardis and the generic telmisartan — treat high blood pressure (hypertension). Bringing blood pressure down lowers the risk of strokes and heart attacks.

Micardis and telmisartan tablets are also approved to reduce the risk of heart attack, stroke, or death from cardiovascular causes in patients 55 or older who are at high cardiovascular risk and cannot take ACE inhibitors. This use is for patients with a history of conditions such as coronary artery disease, peripheral arterial disease, stroke, TIA (mini-stroke), or high-risk diabetes with signs of organ damage.
