Aspirin, Codeine Phosphate, Phenobarbital falls under CNS DEPRESSANTS — the group Sage interacts with.
Some constituents of sage have CNS depressant activity.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
This combo pill packs three ingredients, and two of them already slow down your central nervous system: codeine (an opioid pain reliever) and phenobarbital (a barbiturate sedative). The concern with sage is that some of its natural compounds appear to have mild calming, sedative-like activity in the body too. Stack that on top of two drugs that already cause drowsiness, and in theory you could feel extra sleepy, foggy, dizzy, or slowed down in your breathing.
I'll be honest, this is a theoretical worry based on lab studies, not strong human evidence, and a sprinkle of sage on your dinner almost certainly won't matter. Concentrated sage extracts or the essential oil are where I'd be more cautious. Before adding a sage supplement, run it by your pharmacist or doctor.

CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES
2 of 2Aspirin, Codeine Phosphate, Phenobarbital also falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES.
In vitro evidence suggests that aqueous extracts of sage can inhibit CYP2D6. So far, this interaction has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the wrinkle with this combo product: the codeine part is a prodrug, meaning your body has to switch it on using an enzyme called CYP2D6 before it can relieve pain. Lab studies hint that sage may slow that same enzyme down. If that happened in real life, less codeine would get turned into its active form (morphine), so you might get less pain relief rather than more. The aspirin and phenobarbital pieces aren't really affected by this particular pathway.
I want to be straight with you: this is only seen in test-tube research and has never been reported in actual people, so it's largely theoretical. Cooking with sage won't matter. If you're using concentrated sage extracts or oil regularly, just mention it to your pharmacist or doctor.

Sage is a common kitchen herb that is generally safe in food amounts and is traditionally used for sore throats, digestion, sweating, and memory. Some early research is encouraging for sore throat and menopausal hot flashes, but most uses are not well proven. Concentrated extracts and the essential oil should be used carefully because high doses contain a compound called thujone, which can be harmful.
