Why this applies to Aliskiren

Aliskiren falls under ANTIHYPERTENSIVE DRUGS — the group Sage interacts with.

Theoretically, sage might increase or decrease the effects of antihypertensive drugs.
Animal research suggests that common sage (Salvia officinalis) can cause prolonged blood pressure reduction. However, clinical research suggests that Spanish sage (Salvia lavandulaefolia) can increase blood pressure in some people with hypertension. Until more is known, use with caution.

Here's the honest picture: this one is mostly theoretical, based on animal and lab studies rather than solid human trials. Depending on the type of sage, it could pull in different directions. Common sage (Salvia officinalis) has been shown in animals to lower blood pressure, which could add to what Aliskiren (Tekturna) already does and possibly leave you feeling dizzy, lightheaded, or unusually tired. On the flip side, Spanish sage has nudged blood pressure up in some people, which could work against your medication.

Aliskiren isn't a prodrug, so we're really talking about overlapping effects on blood pressure, not changes to how it's broken down. The sage you cook with is fine. Concentrated extracts or sage oil are where I'd pause. Check with your pharmacist or doctor before adding a sage supplement.

Aliskiren also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, sage might increase the levels and clinical effects of drugs metabolized by CYP3A4.
In vitro evidence suggests that aqueous extracts of sage can inhibit CYP3A4. So far, this interaction has not been reported in humans.

Here's the idea behind this pairing. Aliskiren (Tekturna) is partly cleared by a liver enzyme called CYP3A4. In test-tube studies, water-based sage extracts seem to slow that enzyme down a bit. If that happened in your body, a little more aliskiren could hang around, which in theory might nudge your blood pressure lower or add to side effects like dizziness, lightheadedness, or diarrhea.

I want to keep this in perspective. This is only lab evidence, and it has not actually been reported in people. Sprinkling sage on your food is very unlikely to matter. Concentrated sage capsules or sage essential oil are a different story, so if you use those regularly, just mention it to your pharmacist or doctor so they can keep an eye on your blood pressure.

Sage is a common kitchen herb that is generally safe in food amounts and is traditionally used for sore throats, digestion, sweating, and memory. Some early research is encouraging for sore throat and menopausal hot flashes, but most uses are not well proven. Concentrated extracts and the essential oil should be used carefully because high doses contain a compound called thujone, which can be harmful.

Aliskiren and Tekturna tablets are both approved to treat high blood pressure (hypertension) in adults and in children who are at least 6 years old and weigh 50 kg (about 110 lbs) or more. Lowering high blood pressure reduces the risk of serious events like strokes and heart attacks.

Managing blood pressure is usually part of a broader plan that may also include diet changes, exercise, cholesterol control, and other medications. Many people need more than one blood pressure drug to reach their goal.
