In vitro evidence suggests that aqueous extracts of sage can inhibit CYP2D6. So far, this interaction has not been reported in humans.

This combination product (Theracaps) is a cold and cough remedy, and the piece that matters here is the dextromethorphan, the cough suppressant. Your body clears dextromethorphan using an enzyme called CYP2D6. In lab dish studies, sage extracts seem to slow that enzyme down. If that happened in real life, dextromethorphan could build up a bit, which might mean more drowsiness, dizziness, or feeling foggy.
Here's the reassuring part: this is only seen in test tubes, never reported in actual people, so it's mostly theoretical. Normal cooking amounts of sage are very unlikely to do anything. Concentrated sage extracts or the essential oil are where I'd be more cautious. If you use sage supplements regularly, just run it by your pharmacist or doctor first.

This combo product (Theracaps) is your typical cold and cough mix: acetaminophen for aches and fever, dextromethorphan to quiet a cough, phenylpropanolamine as a decongestant, and pyrilamine as an antihistamine. The idea here is that concentrated sage may slow down a liver enzyme called CYP3A4, which helps clear some of these ingredients. In theory, that could let levels creep up a bit, meaning more drowsiness, dizziness, or dry mouth than you'd expect.
The honest truth is that this is only a lab and test-tube finding. It has never actually been reported in real people, and normal kitchen amounts of sage in your cooking are very unlikely to matter. If you regularly take concentrated sage extract or the essential oil, just mention it to your pharmacist before pairing them.

ANTICHOLINERGIC DRUGS

In vitro evidence suggests that common sage (Salvia officinalis) and Spanish sage (Salvia lavandulaefolia) can inhibit acetylcholinesterase and might increase acetylcholine levels.

Your Theracaps combo packs a few different ingredients, but the one that matters here is pyrilamine, an antihistamine that has drying, anticholinergic effects (think reduced runny nose, but also possible dry mouth and drowsiness). Sage works in the opposite direction: lab studies suggest some sage compounds may raise levels of acetylcholine, the very chemical that antihistamine is trying to dial back.
So in theory, concentrated sage could slightly blunt the drying benefit of the pyrilamine piece. Honestly, this is very weak evidence (test-tube only), and everyday culinary sage in cooking or tea is unlikely to cause any noticeable change. A strong sage extract or essential oil is the bigger question mark. The acetaminophen, dextromethorphan, and phenylpropanolamine parts aren't really involved. If you use sage supplements regularly, just run it by your pharmacist.

This combination product (a cold-and-cough mix with acetaminophen, dextromethorphan, phenylpropanolamine, and pyrilamine) includes acetaminophen, which is partly handled by a liver enzyme called CYP2E1. The concern here is theoretical: in animal studies, drinking sage tea seemed to rev up that enzyme. If the same thing happened in people, acetaminophen could be broken down a bit faster.
Why that matters: CYP2E1 is the pathway that turns a small amount of acetaminophen into a harmful byproduct that can stress the liver. So in theory, speeding it up could matter most if you take a lot of acetaminophen or drink alcohol. Honestly, though, this has never been reported in humans, and normal cooking amounts of sage are very unlikely to do anything. Just check with your pharmacist if you use concentrated sage supplements regularly.

Sage is a common kitchen herb that is generally safe in food amounts and is traditionally used for sore throats, digestion, sweating, and memory. Some early research is encouraging for sore throat and menopausal hot flashes, but most uses are not well proven. Concentrated extracts and the essential oil should be used carefully because high doses contain a compound called thujone, which can be harmful.
