Donepezil (patch) falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Pomegranate interacts with.

Theoretically, pomegranate might increase levels of drugs metabolized by CYP2D6.
In vitro, pomegranate juice inhibits CYP2D6. However, the clinical significance of this potential interaction in humans is not known.

Here's the gist for the donepezil patch (Adlarity): your body partly clears this Alzheimer's medicine using a liver enzyme called CYP2D6. In test-tube studies, pomegranate juice appears to slow that same enzyme down. If that were to happen in real life, donepezil could build up a bit more than expected, and you might notice more of its usual effects, things like nausea, diarrhea, trouble sleeping, dizziness, or a slower heartbeat.

The important part: this comes only from lab work, not from studies in people, so we honestly don't know if it matters at all. Enjoying pomegranate as fruit or a normal glass of juice is unlikely to be a problem. Before using concentrated pomegranate extracts, just run it by your pharmacist or doctor so someone can keep an eye on how you're doing.

Donepezil (patch) also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, pomegranate might increase levels of drugs metabolized by CYP3A4, but most research suggests this interaction is unlikely to be clinically significant.
Pomegranate contains several polyphenols that have individually been shown to inhibit CYP3A4. However, there is contradictory evidence about the effect of whole pomegranate juice on CYP3A4 activity. In vitro, pomegranate juice significantly inhibits the CYP3A4 enzyme, with comparable inhibition to grapefruit juice. In an animal model, pomegranate juice inhibits CYP3A4 metabolism of carbamazepine and increases levels of carbamazepine by 1.5 times; however, in human volunteers, drinking a single glass of pomegranate juice 240 mL or taking 200 mL daily for 2 weeks does not significantly affect levels of the CYP3A4 substrate midazolam after oral or intravenous administration. Another study in healthy volunteers shows that consuming pomegranate juice 300 mL three times daily for three days also does not significantly affect levels of simvastatin, a CYP3A4 substrate This suggests that pomegranate is unlikely to significantly affect levels of CYP3A4 substrates in humans.

Here's the short version: your donepezil patch (Adlarity) is partly cleared by a liver enzyme called CYP3A4, and some compounds in pomegranate can slow that enzyme in the lab. In theory, that could let a bit more donepezil build up, which might mean more of its usual side effects like nausea, diarrhea, dizziness, a slow heart rate, or trouble sleeping.

The good news is that the real-world evidence is reassuring. When researchers gave people pomegranate juice with other CYP3A4 drugs, they didn't see a meaningful change in drug levels. So enjoying pomegranate as a fruit or juice is very unlikely to cause a problem with your patch. Concentrated pomegranate extracts haven't been studied as well, so it's worth mentioning those to your pharmacist or doctor before starting them.
