Here's the simple version: pomegranate juice can gently lower blood pressure on its own. Aliskiren (Tekturna) is already working hard to bring your numbers down, so adding pomegranate could stack on top of it and push your blood pressure a little lower than expected.

If that happens, you might feel dizzy, lightheaded, tired, or wobbly when you stand up quickly. The good news is this is considered a mild and theoretical concern, and an occasional glass of juice is unlikely to cause trouble for most people. I'd be more cautious with concentrated pomegranate extract pills. If you drink it regularly, just keep an eye on your readings and check with your pharmacist or doctor before making any changes.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Aliskiren also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, pomegranate might increase levels of drugs metabolized by CYP3A4, but most research suggests this interaction is unlikely to be clinically significant.
Pomegranate contains several polyphenols that have individually been shown to inhibit CYP3A4. However, there is contradictory evidence about the effect of whole pomegranate juice on CYP3A4 activity. In vitro, pomegranate juice significantly inhibits the CYP3A4 enzyme, with comparable inhibition to grapefruit juice. In an animal model, pomegranate juice inhibits CYP3A4 metabolism of carbamazepine and increases levels of carbamazepine by 1.5 times; however, in human volunteers, drinking a single glass of pomegranate juice 240 mL or taking 200 mL daily for 2 weeks does not significantly affect levels of the CYP3A4 substrate midazolam after oral or intravenous administration. Another study in healthy volunteers shows that consuming pomegranate juice 300 mL three times daily for three days also does not significantly affect levels of simvastatin, a CYP3A4 substrate This suggests that pomegranate is unlikely to significantly affect levels of CYP3A4 substrates in humans.

Here's the short version: pomegranate has some lab compounds that can slow down CYP3A4, an enzyme that helps break down Tekturna (aliskiren). In theory, blocking that enzyme could let a little more of the drug build up, which might nudge your blood pressure lower than expected or add to side effects like dizziness, lightheadedness, or diarrhea.

But the real-world evidence is reassuring. When researchers gave people pomegranate juice and tracked actual CYP3A4 drugs, levels didn't change in any meaningful way. So enjoying pomegranate as food or juice is very unlikely to cause a problem with your aliskiren. If you're thinking about a concentrated pomegranate extract or you notice your blood pressure dropping, just check in with your pharmacist or doctor first.

Aliskiren and Tekturna tablets are both approved to treat high blood pressure (hypertension) in adults and in children who are at least 6 years old and weigh 50 kg (about 110 lbs) or more. Lowering high blood pressure reduces the risk of serious events like strokes and heart attacks.

Managing blood pressure is usually part of a broader plan that may also include diet changes, exercise, cholesterol control, and other medications. Many people need more than one blood pressure drug to reach their goal.
