Pomegranate and Amlodipine

These two interact in 2 distinct ways. Each one is explained separately below.

Why Are There 2 Interactions?

Pomegranate interacts with whole categories of medication. Amlodipine falls into 2 of those categories, so it can interact with Pomegranate in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).

Moderate

ANTIHYPERTENSIVE DRUGS

1 of 2

Why this applies to Amlodipine

Amlodipine falls under ANTIHYPERTENSIVE DRUGS — the group Pomegranate interacts with.

Theoretically, taking pomegranate with antihypertensive drugs might increase the risk of hypotension.
Consuming pomegranate juice can modestly lower blood pressure.
Severity Moderate Likelihood Possible Evidence B

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the simple version: amlodipine already works by relaxing your blood vessels to bring your blood pressure down. Pomegranate, especially as juice, can nudge your blood pressure a little lower on its own. Put the two together and those effects can add up, so your reading may dip more than expected.

If your pressure drops too low, you might feel dizzy, lightheaded, tired, or a bit flushed, especially when you stand up quickly. This isn't a dangerous, well-proven interaction, and an occasional glass of juice or some fruit usually isn't a big deal for most people. Still, if you drink pomegranate juice regularly or use concentrated extracts, mention it to your pharmacist or doctor so they can keep an eye on your numbers.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

2 of 2

Amlodipine also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, pomegranate might increase levels of drugs metabolized by CYP3A4, but most research suggests this interaction is unlikely to be clinically significant.
Pomegranate contains several polyphenols that have individually been shown to inhibit CYP3A4. However, there is contradictory evidence about the effect of whole pomegranate juice on CYP3A4 activity. In vitro, pomegranate juice significantly inhibits the CYP3A4 enzyme, with comparable inhibition to grapefruit juice. In an animal model, pomegranate juice inhibits CYP3A4 metabolism of carbamazepine and increases levels of carbamazepine by 1.5 times; however, in human volunteers, drinking a single glass of pomegranate juice 240 mL or taking 200 mL daily for 2 weeks does not significantly affect levels of the CYP3A4 substrate midazolam after oral or intravenous administration. Another study in healthy volunteers shows that consuming pomegranate juice 300 mL three times daily for three days also does not significantly affect levels of simvastatin, a CYP3A4 substrate This suggests that pomegranate is unlikely to significantly affect levels of CYP3A4 substrates in humans.
Severity Minor Likelihood Unlikely Evidence B

Interaction has been demonstrated in animal or in lab research but has been shown not to occur in humans..

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the quick version: your body uses an enzyme called CYP3A4 to clear amlodipine out of your system. Some of the natural compounds in pomegranate can slow that enzyme in a test tube, and in theory that could let amlodipine build up a bit, which might mean more of its usual effects like dizziness, flushing, ankle swelling, or feeling lightheaded when you stand up.

The good news is that in real people, this just doesn't seem to amount to much. Human studies with pomegranate juice and other CYP3A4 drugs didn't show a meaningful change. So enjoying pomegranate as a food or juice is very unlikely to cause a problem. If you're thinking about a concentrated pomegranate extract, it's worth a quick chat with your pharmacist or doctor first.

About Pomegranate

What is Pomegranate?

Pomegranate
  • Heart and blood vessel health
  • Antioxidant support
  • High blood pressure
  • Inflammation
  • Digestive upset
  • Skin health

Pomegranate is a nutrient-rich fruit that is high in antioxidants and is widely enjoyed as food and juice. Early research suggests it may support heart health and blood pressure, but the evidence is still limited and not strong enough to call it a proven treatment. As a food it is generally safe, but talk to your pharmacist or doctor before using concentrated extracts, especially if you are pregnant, breastfeeding, or take medications.

About Amlodipine

What Is Amlodipine Used For?

Brand name: Norliqva

Amlodipine treats high blood pressure (hypertension) in adults and children aged 6–17 years. Lowering blood pressure reduces the risk of strokes and heart attacks. Sdamlo (oral solution) is specifically indicated for hypertension in adults and pediatric patients 6 years of age and older.

Amlodipine also treats coronary artery disease in adults, including chronic stable angina (chest pain with exertion), vasospastic angina (Prinzmetal's or variant angina, caused by coronary artery spasms), and documented coronary artery disease in patients without heart failure or significantly reduced heart pumping function.
