Here's the simple version: slippery elm turns into a slick, gel-like fiber once it hits water. The thinking is that this gel can coat your gut and trap part of a swallowed pill, so a bit less of the drug gets absorbed into your blood. With terfenadine, that would mean slightly weaker allergy relief, so a stuffy, runny nose or itchy eyes might not be as well controlled.

I'll be honest, this concern is theoretical and based only on how the fiber behaves, not real-world studies, so it may not amount to much. A practical trick is to separate them by a couple of hours. Also worth noting, terfenadine itself was pulled from the U.S. market for heart-rhythm safety reasons, so please run your full medication list by your pharmacist or doctor.
