Flecainide falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Syrian Rue interacts with.

Theoretically, Syrian rue might increase the levels and clinical effects of drugs metabolized by CYP2D6.
Syrian rue contains beta-carboline alkaloids, including harmine, harmaline, harmalol, harmol, and harmane. In vitro research suggests that these beta-carboline alkaloids can inhibit CYP2D6 enzymes.

Here's the situation with these two. Your body relies on a liver enzyme called CYP2D6 to clear flecainide out of your system. Compounds in Syrian Rue (the harmala alkaloids) can slow that same enzyme down, at least in lab studies. If flecainide is cleared more slowly, it can build up in your blood, and flecainide is one of those drugs where the safe window is narrow.

Higher-than-expected flecainide levels can cause dizziness, blurred vision, trouble breathing, and, more worryingly, new or worse heart rhythm problems, which is the very thing this drug is meant to control. To be fair, this concern is theoretical and comes from test-tube data, not real patients. Still, with a heart medication like this, please loop in your doctor or pharmacist before mixing the two.

What is Syrian Rue?

Syrian Rue (Peganum harmala) is a traditional plant whose seeds contain potent compounds called harmala alkaloids, including MAO-inhibiting harmine and harmaline. There is little reliable human evidence that it safely treats any condition, and it carries real risks of toxicity, so it is not recommended for self-treatment. About Flecainide

What Is Flecainide Used For?

Brand name: Tambocor

Flecainide Acetate tablets are used to prevent episodes of certain abnormal heart rhythms. In patients without structural heart disease, it is approved to prevent paroxysmal supraventricular tachycardia (PSVT — episodes of rapid heart rate starting above the ventricles) and paroxysmal atrial fibrillation/flutter (PAF — intermittent episodes of irregular, rapid heart rhythm in the upper chambers), when these cause disabling symptoms.

Flecainide Acetate tablets are also used to prevent life-threatening ventricular arrhythmias — dangerous rapid rhythms starting in the lower chambers of the heart, such as sustained ventricular tachycardia. It is not recommended for less serious ventricular arrhythmias, for patients with a recent heart attack, or for chronic (ongoing, persistent) atrial fibrillation.
