Thyme and Tripelennamine

Here is the interaction we found between these two, with the evidence behind it.

Moderate

ANTICHOLINERGIC DRUGS

Why this applies to Tripelennamine

Tripelennamine falls under ANTICHOLINERGIC DRUGS — the group Thyme interacts with.

Theoretically, concurrent use of anticholinergic drugs and thyme essential oil might reduce the effects of anticholinergic drugs.
In vitro evidence suggests that thyme essential oil and specific essential oil constituents like thymohydroquinone and carvacrol can inhibit acetylcholinesterase (AChE). However, this effect has not been observed in humans.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here is the gist for Tripelennamine, which is an older antihistamine that also has anticholinergic (drying) properties. Concentrated thyme essential oil, in test-tube studies, can block an enzyme called acetylcholinesterase. Blocking that enzyme leaves more of the messenger chemical acetylcholine around, which works in the opposite direction of an anticholinergic medicine. In theory, that could slightly blunt some of Tripelennamine's effects, including how well it dries up a runny nose or calms itching.

Honestly, I would not lose sleep over this one. The evidence is only lab-based and has not been seen in real people, and ordinary culinary thyme is nowhere near strong enough to matter. If you plan to use concentrated thyme oil regularly, just run it by your pharmacist first.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)

From our pharmacist’s overview on the Tripelennamine page

“People reaching for tripelennamine (PBZ) usually want relief from allergies or stubborn itching, and this older antihistamine already dries the mouth, blurs vision, and clouds thinking on its own. I would treat its supplement list with real care, starting with the three major-rated entries.”

About Thyme

What is Thyme?

Thyme
  • Cough and respiratory congestion
  • Sore throat
  • Digestive upset
  • Antimicrobial and antiseptic uses
  • Flavoring in cooking

Thyme is a common kitchen herb that has long been used for coughs, sore throats, and digestive complaints. It is generally safe in the amounts found in food, and some cough products that combine thyme with other herbs have modest supporting evidence, but high-quality research on thyme alone is limited. The concentrated essential oil should never be swallowed undiluted.
