Toothed Clubmoss and Fluphenazine

Here is the interaction we found between these two, with the evidence behind it.

Moderate

ANTICHOLINERGIC DRUGS

Why this applies to Fluphenazine

Fluphenazine falls under ANTICHOLINERGIC DRUGS — the group Toothed Clubmoss interacts with.

In animal models, toothed clubmoss and huperzine A, an active constituent of toothed clubmoss, reversed cognitive deficits induced by scopolamine. Theoretically, concurrent use of anticholinergic drugs and toothed clubmoss might decrease the effectiveness of toothed clubmoss or the anticholinergic drug.
Some anticholinergic drugs include atropine, benztropine (Cogentin), biperiden (Akineton), procyclidine (Kemadrin), and trihexyphenidyl (Artane).
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the basic tug-of-war: Toothed Clubmoss (through huperzine A) raises levels of acetylcholine in the brain, while Fluphenazine (Prolixin) has some anticholinergic activity that does the opposite by blocking acetylcholine. Because they push in opposite directions, taking them together could blunt the memory benefit you're hoping to get from the clubmoss, and in theory could slightly work against fluphenazine too.

I want to be honest about how soft this evidence is. It comes only from animal studies, so it's mostly theoretical. I wouldn't expect dramatic changes like a return of strong hallucinations or new muscle stiffness, but quality of these supplements varies a lot. Before combining them, please check with your pharmacist or doctor so they can weigh it for your situation.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).
  • BCase-control study.
  • CConsensus.

From our pharmacist’s overview on the Fluphenazine page

“Take real care with supplements while on fluphenazine (Prolixin), an antipsychotic used mainly for schizophrenia. Five of the 148 ingredients on file rate major: herbs like henbane that stack anticholinergic side effects (dry mouth, blurred vision, confusion) onto a drug already prone to them, two rauwolfia plants that can intensify it…”

About Toothed Clubmoss

What is Toothed Clubmoss?

Toothed Clubmoss
  • Memory and cognitive support
  • Alzheimer's disease and dementia
  • General brain or 'nootropic' support
  • Inflammation and pain (traditional use)
  • Age-related memory loss

Toothed Clubmoss is a moss-like plant best known as the natural source of huperzine A, a compound studied mainly for memory and Alzheimer's disease. Some early research is promising, but the evidence is limited and the supplement is not well standardized or regulated, so quality between products varies. Because it can affect the nervous system, talk with a pharmacist or doctor before using it.
