Theoretically, xanthan gum can alter the absorption of oral drugs due to its fiber qualities. Xanthan gum slows gastric emptying and has been used to control the release of drugs in tablet formulations. To avoid any alterations in drug absorption, xanthan gum should be taken 30-60 minutes after oral medications.

Here's the gist with Zydelig (idelalisib) and xanthan gum. Because xanthan gum is a soluble fiber that turns into a gel in your stomach, it can slow how fast things empty out and, in theory, change how much of a swallowed pill your body soaks up. This isn't proven for idelalisib specifically, it's a general concern based on how fiber behaves, so I wouldn't lose sleep over it.

To play it safe and keep your absorption steady, a simple trick is to take your idelalisib first and wait about 30 to 60 minutes before having any concentrated xanthan gum (like thickeners or fiber supplements). Tiny food amounts are unlikely to matter. Since idelalisib is a serious cancer drug, run any thickener or fiber product by your pharmacist or oncologist first.

What Is Idelalisib Used For?

Brand name: Zydelig

Zydelig (idelalisib) is approved to treat relapsed chronic lymphocytic leukemia (CLL) — a cancer of the blood and bone marrow — when used together with rituximab. It is specifically for patients who have already received treatment and for whom rituximab alone would be appropriate due to other health conditions.

Zydelig is not approved or recommended as a first-line (initial) treatment for CLL or any other cancer, including small lymphocytic lymphoma (SLL) or follicular lymphoma (FL). It is also not approved in combination with bendamustine and rituximab, or with rituximab alone, for FL, SLL, or other slow-growing non-Hodgkin lymphomas.
