Because xanthan gum forms a thick gel in your gut and slows how fast things move through your stomach, it could in theory change how much of your penbutolol gets absorbed. The concern is mostly about timing: if the two are sitting together, the gel may trap a bit of the medicine or slow it down.
To be clear, this is a theoretical worry based on how fiber behaves, not something proven to cause problems. The simple fix is to take your penbutolol first, then wait about 30 to 60 minutes before having concentrated xanthan gum. Small food amounts are unlikely to matter. If you use thickened liquids regularly or notice your blood pressure or heart rate acting differently, check with your pharmacist or doctor.
