Our pharmacy team’s full take, with four database checks built into the cards below — a summary of what is known, not a grade of the product itself.
What’s inside
Full disclosure
This product contains one active ingredient: acetyl-L-carnitine hydrochloride at 500 mg per capsule. Acetyl-L-carnitine is a compound your body naturally makes; it helps cells produce energy and is involved in brain function.
The capsule itself is made from hypromellose (a plant-based coating) and contains inactive ingredients including cellulose, silicon dioxide, and magnesium stearate as binders and flow agents.
Does it work?
Moderate evidence
The evidence we hold rates acetyl-L-carnitine as possibly effective for several conditions: age-related cognitive decline, Alzheimer disease, alcohol use disorder, diabetic neuropathy, and depression. That means some research supports its use for these purposes, but the evidence isn't definitive.
If you're considering this supplement for one of those reasons, talk with your doctor about whether it makes sense alongside whatever else you're doing.
How safe is it?
Well-documented data
Acetyl-L-carnitine is generally well tolerated in the short term, though long-term safety isn't fully established. The most common side effects are agitation, dry mouth, headache, insomnia, and reduced appetite.
A few people report a fishy odor to their urine, breath, or sweat—that's a harmless metabolite. Some experience nausea, vomiting, diarrhea, or stomach upset, though these don't occur more often than with placebo in studies.
There isn't enough reliable safety information for pregnancy, so it's best avoided during pregnancy unless your doctor advises otherwise. Safety while breastfeeding is unknown; the safer choice is to avoid it unless your doctor says it's okay.
Meds to double-check
Moderate interaction found
Before you take this product, check with your pharmacist if you're on serotonergic drugs (antidepressants, migraine medications—a moderate concern), blood thinners like warfarin or acenocoumarol (moderate), or thyroid hormone replacement (moderate). No interactions are documented for ingredients we could not check, because this product has only one active ingredient.
The bottom line
Scorecard at a glanceFully disclosed formula with some supporting evidence for its stated purpose. Moderate medication interactions have been identified, and safety information is well characterized.
Acetyl-L-carnitine may help with cognitive decline, Alzheimer disease, neuropathy, depression, or alcohol use disorder, but the evidence is moderate. If you take blood thinners, antidepressants, migraine drugs, or thyroid replacement, you need to check this product with your pharmacist first.

Acetyl L-Carnitine 500 mg by Bluebonnet Drug Interactions
HelloPharmacist Interaction Report
Acetyl L-Carnitine 500 mg by Bluebonnet does interact with several medication types.
The ingredient acetyl-L-carnitine may increase serotonin activity in the brain, which poses a moderate risk of serotonergic side effects—including serotonin syndrome and cerebral blood vessel constriction—when taken alongside serotonergic drugs like antidepressants and migraine treatments.
Read the full breakdown — every affected drug type, severity by severity
It also carries moderate concerns with blood thinners. Acetyl-L-carnitine may enhance the effects of warfarin (Coumadin) and acenocoumarol (Sintrom), potentially raising your bleeding risk.
Additionally, it may interfere with thyroid hormone replacement therapy by blocking thyroid hormone from entering cells, which could reduce how well your thyroid medication works.
Altogether, these interactions span 203 individual medications. If you take any blood thinner, antidepressant, migraine medication, or thyroid hormone, check your exact drugs with the tool on this page before you start.

For each ingredient in Acetyl L-Carnitine 500 mg with known interactions, here are the types of medications it can affect. Open any type for the detail — or search your exact drug in the checker above.
Acetyl-L-Carnitine Hydrochloride
Acenocoumarol (Sintrom)
Theoretically, acetyl-L-carnitine might increase the anticoagulant effects of acenocoumarol.
L-carnitine, the parent compound of acetyl-L-carnitine, might enhance the anticoagulant effects of acenocoumarol, an oral anticoagulant that is similar to warfarin, but shorter-acting. There are at least two case reports of INR elevation when L-carnitine was taken with acenocoumarol. In one case, a 33-year-old male with a previously stable INR had an elevated INR of 4.65 after L-carnitine was started and continued for 10 weeks. INR normalized after discontinuation of the L-carnitine-containing product. It is unclear if such an interaction would also occur with acetyl-L-carnitine.
Serotonergic Drugs
Theoretically, acetyl-L-carnitine might increase the risk of serotonergic side effects, including serotonin syndrome and cerebral vasoconstrictive disorders, when taken with serotonergic drugs.
Animal research shows that acetyl-L-carnitine can increase levels of serotonin in the brain.
Thyroid Hormone
Theoretically, acetyl-L-carnitine might decrease the effectiveness of thyroid hormone replacement.
L-carnitine appears to act as a peripheral thyroid hormone antagonist by inhibiting entry of thyroid hormone into the nucleus of cells. Taking L-carnitine also seems to diminish some of the symptoms of hyperthyroidism. It is unclear if such an interaction would occur with acetyl-L-carnitine.
Warfarin (Coumadin)
Theoretically, acetyl-L-carnitine might increase the anticoagulant effects of warfarin.
L-carnitine, the parent compound of acetyl-L-carnitine, might increase the anticoagulant effects of acenocoumarol, a shorter-acting oral anticoagulant similar to warfarin. There is not enough information to know whether this interaction occurs with acetyl-L-carnitine and warfarin.
