The piece of this combo cold product that matters here is the dextromethorphan (the cough suppressant). Dextromethorphan can nudge serotonin levels up in the brain. Acetyl-L-carnitine may do something similar, based only on animal studies, so on paper, stacking the two could add up to a little extra serotonin activity.

In rare cases too much serotonin can cause something called serotonin syndrome, with signs like agitation, a racing heart, sweating, shivering, muscle twitching, or feeling jittery. I want to be honest though: this is a theoretical concern with weak evidence, and most people using a short course of this cold medicine won't notice anything. The acetaminophen and chlorpheniramine pieces aren't the issue here. Still, it's worth a quick chat with your pharmacist, especially if you take other serotonin-affecting meds.
