Strong evidence
Coenzyme Q10 is likely effective for replacing coenzyme Q10 when your body is deficient. It's also possibly effective for fibromyalgia, migraine headache, congestive heart failure, and diabetic neuropathy.
The evidence for these conditions isn't as strong as for deficiency, but clinical research supports its use.
How safe is it?
Well-documented data
Orally, coenzyme Q10 is generally well tolerated. The most common side effects are gastrointestinal — appetite suppression, diarrhea, epigastric discomfort, heartburn, nausea, and vomiting — and these occur in less than 1% of users.
Dividing doses above 100 mg can help minimize these. Headache, dizziness, and insomnia have been reported rarely.
Skin itching and allergic rash are uncommon. During pregnancy, there isn't enough reliable safety data, so it's best to avoid unless your doctor advises otherwise.
Safety while breastfeeding hasn't been well studied, so check with your doctor before use.

Active-Q 50 mg contains ubiquinol (the active form of coenzyme Q10), which interacts with a small number of medication types.
The most serious concern is with chemotherapy drugs called alkylating agents (such as cyclophosphamide). Coenzyme Q10 works as an antioxidant, and theoretically this could reduce how well these cancer medications work, since they rely on generating free radicals to kill tumor cells.

A second Moderate-severity interaction involves warfarin (Coumadin), a blood thinner. Coenzyme Q10 is chemically similar to vitamin K, which promotes clotting.
Four cases have been reported where coenzyme Q10 appeared to decrease warfarin's effectiveness. If you take warfarin, your doctor may need to adjust your dose or monitor your blood clotting more closely.
There's also a Minor interaction with blood pressure medications. Some research shows coenzyme Q10 can lower blood pressure on its own, so combined with antihypertensive drugs it might lower your pressure further than intended.

Ubiquinol
Alkylating Agents
Coenzyme Q10 has antioxidant effects. Theoretically, this may reduce the activity of chemotherapy drugs that generate free radicals.
Theoretically, antioxidants such as coenzyme Q10 might protect tumor cells from chemotherapeutic agents that work by inducing oxidative stress, such as alkylating agents (e.g., cyclophosphamide) and radiation therapy. The clinical importance of this interaction is unknown.
Warfarin (Coumadin)
Coenzyme Q10 is chemically similar to menaquinone and might have vitamin K-like procoagulant effects, which could decrease the effects of warfarin.
Concomitant use of coenzyme Q10 and warfarin might reduce the anticoagulant effects of warfarin. Four cases of decreased warfarin efficacy thought to be due to coenzyme Q10 have been reported. However, there is some preliminary clinical research that suggests coenzyme Q10 might not significantly decrease the effects of warfarin in patients who have a stable INR.
Antihypertensive Drugs
Theoretically, coenzyme Q10 might have additive effects with antihypertensive drugs.
Some clinical research shows that coenzyme Q10 can significantly lower blood pressure, although other studies have shown conflicting results.
