Peppermint in this product has the strongest evidence: it's rated Likely Effective for irritable bowel syndrome (IBS), and Possibly Effective for dyspepsia (indigestion), nausea from chemotherapy, colon spasm during barium enema, and side effects from endoscopy. Apple cider vinegar, however, has insufficient reliable evidence to rate its effectiveness for acne, athletic performance, cognitive function, dental plaque, depression, or eczema.

Well-documented data
In food amounts, apple cider vinegar is well tolerated. When used as a supplement for short-term medicinal purposes, it also appears well tolerated — but long-term use in larger amounts may be unsafe.
Serious rare effects from chronic high-dose use include low potassium, elevated renin levels, and bone loss (osteoporosis). One documented case involved 250 mL daily for 6 years.
Topically, apple cider vinegar can cause chemical burns and skin irritation. Peppermint leaf tea and culinary amounts are generally safe.
The most common side effects from oral use are abdominal pain, anal burning, belching, diarrhea, dry mouth, heartburn, nausea, and vomiting. Rare cases of anaphylaxis (severe allergic reaction) have been reported.
Pregnancy and lactation: there isn't enough data on file for apple cider vinegar to say either way — talk with your doctor or pharmacist. Peppermint is rated Likely Safe in pregnancy and lactation.

With insulin, there's a theoretical risk of low blood sugar (hypoglycemia). Peppermint in this product may slow how your liver clears certain medications — specifically drugs processed through CYP2C19, CYP2C9, CYP3A4, and cyclosporine — and cause them to build up in your bloodstream.

Peppermint (Mentha x piperita) leaf extract 10:1
Cyclosporine (Neoral, Sandimmune)
Theoretically, peppermint oil might increase the levels and adverse effects of cyclosporine.
In animal research, peppermint oil inhibits cyclosporine metabolism and increases cyclosporine levels. Inhibition of cytochrome P450 3A4 (CYP3A4) may be partially responsible for this interaction. An interaction between peppermint oil and cyclosporine has not been reported in humans.
Cytochrome P450 2C19 (Cyp2C19) Substrates
Theoretically, peppermint might increase the levels of CYP2C19 substrates.
In vitro research shows that peppermint oil inhibits CYP2C19. So far, this interaction has not been reported in humans.
Cytochrome P450 2C9 (Cyp2C9) Substrates
Theoretically, peppermint might increase the levels of CYP2C9 substrates.
In vitro research shows that peppermint oil inhibits CYP2C9. So far, this interaction has not been reported in humans.
Cytochrome P450 3A4 (Cyp3A4) Substrates
Theoretically, peppermint might increase the levels of CYP3A4 substrates.
Clinical research in healthy volunteers shows that a single dose of peppermint oil 600 mg inhibits CYP3A4 enzymes and increases the AUC of felodipine, a CYP3A4 substrate. However, in vitro research suggests that peppermint oil only inhibits CYP3A4 at very high concentrations.
Cytochrome P450 1A2 (Cyp1A2) Substrates
Theoretically, peppermint might increase the levels of CYP1A2 substrates.
In vitro and animal research shows that peppermint oil and peppermint leaf inhibit CYP1A2. However, in clinical research, peppermint tea did not significantly affect the metabolism of caffeine, a CYP1A2 substrate. It is possible that the 6-day duration of treatment may have been too short to identify a difference.
Apple Cider Vinegar
Digoxin (Lanoxin)
Theoretically, concomitant use of apple cider vinegar with digoxin might increase the risk of cardiac toxicity.
A case of hypokalemia related to chronic use of apple cider vinegar has been reported. Theoretically, overuse of apple cider vinegar could decrease potassium levels, increasing the risk of toxicity with digoxin.
Diuretic Drugs
Theoretically, concomitant use of apple cider vinegar with diuretic drugs might increase the risk of hypokalemia.
A case of hypokalemia related to chronic use of apple cider vinegar has been reported. There is some concern that people taking apple cider vinegar along with potassium depleting diuretics might have an increased risk for hypokalemia.
Insulin
Theoretically, concomitant use of apple cider vinegar with insulin might increase the risk of hypokalemia.
A case of hypokalemia related to chronic use of apple cider vinegar has been reported. Theoretically, overuse of apple cider vinegar concomitantly with insulin might increase the risk of hypokalemia.
Antidiabetes Drugs
Theoretically, taking apple cider vinegar with antidiabetes drugs might increase the risk of hypoglycemia.
Apple cider vinegar might reduce fasting and postprandial blood glucose levels and decrease gastric emptying in people with diabetes. However, not all research agrees. Theoretically, it might have additive effects on glucose levels when used with antidiabetes drugs.
