Acetaminophen, Chlorpheniramine, Dextromethorphan, Pseudoephedrine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Peppermint interacts with.

Theoretically, peppermint might increase the levels of CYP3A4 substrates.
Clinical research in healthy volunteers shows that a single dose of peppermint oil 600 mg inhibits CYP3A4 enzymes and increases the AUC of felodipine, a CYP3A4 substrate. However, in vitro research suggests that peppermint oil only inhibits CYP3A4 at very high concentrations.

Here's the gist for this cold-and-cough combo. Concentrated peppermint oil may gently slow down a liver enzyme called CYP3A4, which helps break down a couple of the ingredients in this product, mainly dextromethorphan (the cough suppressant). In theory, if that ingredient clears more slowly, levels could nudge upward, meaning a bit more drowsiness, dizziness, or that fuzzy, lightheaded feeling. The chlorpheniramine can already make you sleepy, so any added effect could pile on.

The good news: this is mostly theoretical and based on weak evidence, and a normal cup of peppermint tea or a mint isn't likely to matter. Concentrated peppermint oil capsules are the bigger question. If you're using those regularly, just run it by your pharmacist before combining.

Theoretically, peppermint might increase the levels of CYP1A2 substrates.
In vitro and animal research shows that peppermint oil and peppermint leaf inhibit CYP1A2. However, in clinical research, peppermint tea did not significantly affect the metabolism of caffeine, a CYP1A2 substrate. It is possible that the 6-day duration of treatment may have been too short to identify a difference.

Here's the gist with this cold and cough combo. The idea on file is that peppermint might block a liver enzyme called CYP1A2, which could in theory slow how your body clears certain medicines. For this product, that effect would be small at most. The acetaminophen, chlorpheniramine, and pseudoephedrine parts mostly rely on other pathways, so peppermint is unlikely to change them much.

Honestly, the evidence here is weak. When researchers gave people peppermint tea, it didn't meaningfully change how they processed caffeine (another CYP1A2 drug). If anything theoretically nudged up, you might feel a bit more drowsiness or dry mouth, but that's not well proven. Enjoy your peppermint, and just check with your pharmacist if you take this combo often.
