Peppermint and Aspirin, Pentazocine

Here is the interaction we found between these two, with the evidence behind it.

Minor

CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES

Why this applies to Aspirin, Pentazocine

Aspirin, Pentazocine falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Peppermint interacts with.

Theoretically, peppermint might increase the levels of CYP1A2 substrates.
In vitro and animal research shows that peppermint oil and peppermint leaf inhibit CYP1A2. However, in clinical research, peppermint tea did not significantly affect the metabolism of caffeine, a CYP1A2 substrate. It is possible that the 6-day duration of treatment may have been too short to identify a difference.
Severity Minor Likelihood Possible Evidence B

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist with Talwin Compound (a mix of aspirin and pentazocine) and peppermint. In lab dishes and animal studies, peppermint can mildly slow down a liver enzyme called CYP1A2, which helps process pentazocine. If that enzyme runs slower, pentazocine could in theory build up a bit and lead to more of its usual effects, like drowsiness, dizziness, nausea, or feeling lightheaded.

But please don't lose sleep over this. This is mostly theoretical. In actual human studies, peppermint tea didn't meaningfully change how the body cleared a similar CYP1A2 drug. Food and tea amounts of peppermint are very unlikely to cause trouble. Concentrated peppermint oil capsules are a bigger unknown. If you take those regularly, mention it to your pharmacist or doctor.

From our pharmacist’s overview on the Aspirin, Pentazocine page

“Talwin Compound pairs pentazocine, an opioid painkiller, with aspirin, and I read its supplement list with more concern than most. The great majority of entries do one of two things: raise the bleeding risk that aspirin already carries, or deepen the sedation pentazocine causes.”
