Beet root extract is possibly effective for athletic performance and for easing exercise-induced muscle soreness based on the evidence we hold. For chronic obstructive pulmonary disease (COPD), it's rated possibly ineffective.
For cholesterol levels (dyslipidemia) and brain function (cognitive function), the evidence isn't strong enough to rate — meaning we don't have reliable data to say whether it works.
How safe is it?
Well-documented data
Beet is widely eaten as food and is generally well tolerated by most adults. Concentrated supplements, though, lack the long-term safety data that food has, so use caution if you're taking them regularly.
The most common side effect is harmless red or dark discoloration of stools and urine. In rare cases, very large amounts may cause low blood calcium (hypocalcemia) or kidney damage.
One case report described a patient on blood thinners who had black stools after taking beet pills, though upper gastrointestinal bleeding was ruled unlikely. There isn't enough safety data on concentrated beet supplements during breastfeeding, so talk with your doctor or pharmacist for personalized advice.

Beet Root Extract
Cytochrome P450 3A4 (Cyp3A4) Substrates
Theoretically, beet might increase the levels of CYP3A4 substrates.
In vitro research suggests that betanin, the major pigment in beet, competitively inhibits CYP3A4 in a dose-dependent manner similarly to strong CYP3A4 inhibitor ketoconazole.
Antihypertensive Drugs
Beet and beetroot contain nitrates, which can cause vasodilation, potentially leading to lower blood pressure. However, a study published in the European Journal of Clinical Nutrition using concentrated beetroot juice found no significant impact on blood pressure or heart rate in different age groups. Other small clinical studies suggest that while beet consumption might transiently lower blood pressure due to vessel dilation, there's no consistent evidence of a lasting effect. Overall, the theoretical risk of reduced blood pressure due to beet's nitrate content exists, but studies generally indicate a low and temporary impact rather than a sustained decrease.
Cytochrome P450 1A2 (Cyp1A2) Substrates
Theoretically, beet might decrease the levels and clinical effects of CYP1A2 substrates.
In vitro research suggests that beet induces CYP1A2 enzymes.
