Benfotiamine is generally well tolerated in the studies we have, but long-term safety hasn't been fully worked out. In one 6-week study of 98 people taking 300–600 mg daily, a small number reported allergic skin reactions and a small number reported mild stomach discomfort — but these rates were no higher than placebo.

There isn't enough safety information to recommend it during pregnancy, so your doctor should weigh the pros and cons if you're pregnant or planning to be. Safety while breastfeeding hasn't been well studied, so caution is advised if you're nursing.
