What’s inside
Full disclosure
This product contains one active ingredient: L-carnitine, supplied here as L-carnitine fumarate with additional carnitine forms (Biosint Carnitines). The capsule also holds inactive ingredients — modified cellulose, magnesium stearate, and silicon dioxide — which are fillers and binders.
L-carnitine is a compound your body uses to move fats into cells so they can be burned for energy. You make some naturally, but supplementation is used when levels are low or in certain heart and metabolic conditions.
Does it work?
Moderate evidence
The evidence for L-carnitine is strongest for treating L-carnitine deficiency itself, where it is considered effective. For high cholesterol (hyperlipidemia), heart failure (congestive heart failure), and angina, it is rated possibly effective — meaning there's some clinical support but it's not definitive.
The data we hold doesn't establish whether this product works for other uses.
How safe is it?
Well-documented data
L-carnitine is generally well tolerated at typical doses. The most common side effects are gastrointestinal — stomach cramps, pain, diarrhea, nausea, heartburn, and reduced appetite — and a fishy body odor has been reported.
At higher doses, stomach upset is more likely. Rarely, seizures have been reported.
Research shows that oral supplementation with L-carnitine does not increase cardiovascular risk and may even reduce all-cause mortality and certain arrhythmias. Safety data for pregnancy is not on file in the information we hold.
Lactation is rated possibly safe, though safety while breastfeeding is not well studied, so caution is advised. If you are pregnant or breastfeeding, talk with your doctor or pharmacist before taking this supplement.
Meds to double-check
Moderate interaction found
Check with your doctor or pharmacist before taking this if you use blood thinners (anticoagulants) such as warfarin or acenocoumarol — L-carnitine may increase their anticoagulant effects and raise bleeding risk. Also confirm it's safe if you take thyroid hormone replacement, as L-carnitine may reduce its effectiveness.
The bottom line
Scorecard at a glanceFully disclosed formula with some supporting evidence for its stated purpose. Moderate medication interactions have been identified, and safety information is well characterized.
L-carnitine is a good choice if you have a documented L-carnitine deficiency or want to explore it for heart health or cholesterol under medical guidance. Be cautious if you take blood thinners or thyroid replacement — those need a conversation with your doctor first.
Expect the possibility of mild stomach upset or a fishy odor at higher doses. Check with your own doctor or pharmacist before adding this to your routine, especially if you're on any medications.

General Statements
L-Carnitine is an amino acid found abundantly in heart and skeletal muscles. It promotes energy production in cells by transporting fatty acids into mitochondria, intracellular structures that generate metabolic energy in the form of ATP. In this role, L-Carnitine helps the body metabolize fat efficiently. Fumarate is a naturally occurring compound in the body. Fumarate serves as an intermediate in the Krebs cycle, a key energy-producing process that takes place in mitochondria. L-Carnitine helps promote oxygenation of heart muscle and may enhance exercise recovery.

HelloPharmacist Interaction Report
L-Carnitine Fumarate With Biosint Carnitines by Doctor's Best does interact with medications, chiefly through its L-carnitine content.
The most serious concerns are blood thinners (anticoagulants) — specifically warfarin and acenocoumarol — where L-carnitine may increase their effects and raise bleeding risk. There are at least two case reports of INR elevation (a measure of how thin your blood is) when L-carnitine was taken alongside acenocoumarol.

L-carnitine also interacts with thyroid hormone replacement therapy. It appears to block thyroid hormone from entering cells and diminishing its effectiveness, which means your dose might not work as intended.
Altogether, these interactions span 19 individual medications.
Before you start this supplement, check with your doctor or pharmacist about any blood thinners or thyroid medication you take. They can tell you whether it's safe to combine them or if timing and monitoring are needed.

L-Carnitine Fumarate
Acenocoumarol (Sintrom)
Theoretically, L-carnitine might increase the anticoagulant effects of acenocoumarol.
L-carnitine might enhance the anticoagulant effects of acenocoumarol, an oral anticoagulant similar to warfarin, but shorter-acting. There are at least two case reports of INR elevation with concomitant use. In one case, a 33-year-old male with a previously stable INR had an elevated INR of 4.65 after L-carnitine was started and continued for 10 weeks. INR normalized after discontinuation of the L-carnitine-containing product.
Thyroid Hormone
Theoretically, L-carnitine might decrease the effectiveness of thyroid hormone replacement.
L-carnitine appears to act as a peripheral thyroid hormone antagonist by inhibiting entry of thyroid hormone into the nucleus of cells. Taking L-carnitine also seems to diminish some of the symptoms of hyperthyroidism.
Warfarin (Coumadin)
Theoretically, L-carnitine might increase the anticoagulant effects of warfarin.
L-carnitine might increase the anticoagulant effects of acenocoumarol, a shorter-acting oral anticoagulant similar to warfarin. There is not enough information to know whether this interaction occurs with L-carnitine and warfarin.
