HelloPharmacist Scorecard of Black Cherry 450 mg 4:1 Extract by TerraVita Premium Collection
Our pharmacy team’s full take, with four database checks built into the cards below — a summary of what is known, not a grade of the product itself.
What’s inside
Full disclosure
This product contains one active ingredient: Black Cherry extract at 450 mg per capsule in a 4:1 concentration. The capsule itself is made from gelatin and water, which are inactive ingredients that hold the extract together.
Does it work?
Couldn't assess
We don't have effectiveness data on file for this Black Cherry extract, so we can't tell you whether the evidence supports its use for any particular purpose. If you're considering it for a specific reason, ask your pharmacist what the research shows for that use.
How safe is it?
Well-documented data
Black Cherry extract is generally considered safe in small amounts as it appears in over-the-counter cough products, but the plant itself contains compounds that can form cyanide in your body. Large amounts carry real risk — there's a documented case of a woman who consumed roughly 300 grams of wild cherries steeped in alcohol and developed severe poisoning with headache, nausea, vomiting, confusion, difficulty breathing, and eventually loss of consciousness.
The safety data advises against using this product during pregnancy (rated Likely Unsafe) and while breastfeeding, because there isn't enough information to know it's safe in those situations. If you're pregnant, planning to become pregnant, or breastfeeding, talk with your doctor or pharmacist before using it.
Meds to double-check
Moderate interaction found
Before you take this product, double-check any cholesterol medications like lovastatin or antifungals like ketoconazole and itraconazole — these are metabolized by the same liver enzyme the cherry extract may inhibit, and while the interaction hasn't been seen in people yet, it's a Moderate-severity concern worth confirming with your pharmacist.
The bottom line
Scorecard at a glanceFully disclosed formula with no assessable stated purpose. Moderate medication interactions have been identified, and safety information is well characterized.
This single-ingredient product is marketed as a concentrated cherry extract. It hasn't been studied for effectiveness in our data, and the safety profile carries caution because of cyanide-forming compounds at higher doses.
The one documented interaction (with CYP3A4 drugs) is theoretical so far in humans. Check your medications with the tool on this page before starting, and talk to your pharmacist if you're pregnant, breastfeeding, or taking a cholesterol or antifungal medication.

HelloPharmacist Interaction Report
Black Cherry 450 mg 4:1 Extract by TerraVita Premium Collection has one documented interaction to watch.
The cherry extract may inhibit a liver enzyme (cytochrome P450 3A4, or CYP3A4) that breaks down many medications. In theory, this could raise the blood levels of drugs processed by that enzyme — but so far, this interaction hasn't been reported in actual people, only in lab studies.
Read the full breakdown — every affected drug type, severity by severity
Altogether, these interactions span 633 individual medications. The affected drugs include cholesterol medications like lovastatin, antifungals like ketoconazole and itraconazole, and many others metabolized by CYP3A4.
Because the interaction is labeled Moderate and hasn't been seen in humans yet, the real-world risk is unclear — but it's worth checking your exact medications with the search tool on this page before you start.

Cytochrome P450 3A4 (Cyp3A4) Substrates
In vitro research suggests that wild cherry can inhibit cytochrome P450 3A4 (CYP3A4) enzymes. Theoretically, wild cherry might increase levels of drugs metabolized by CYP3A4. However, so far, this interaction has not been reported in humans.
Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), ketoconazole (Nizoral), itraconazole (Sporanox), fexofenadine (Allegra), triazolam (Halcion), and others.
