How safe is it?

Wormwood Extract carries real safety concerns. It contains thujone, a neurotoxin that can cause muscle aches, nausea, and vomiting even in typical medicinal amounts; at higher doses or with chronic use, it's linked to kidney damage, muscle breakdown, seizures, and serious neurological effects including hallucinations, tremors, and brain damage.
Pau d'Arco Extract is generally well tolerated at lower doses, but its active compound lapachol can cause anemia, bleeding risk, and severe nausea or diarrhea when doses exceed 1.5 grams daily. Wormwood is unsafe during pregnancy—it may stimulate the uterus—and safety data is insufficient for breastfeeding.
Pau d'Arco should be avoided during pregnancy due to possible harm to the developing baby, and there isn't enough safety information for breastfeeding. Short-term use of properly prepared wormwood products may be acceptable, but human safety data for pau d'arco is limited.

Theoretically, pau d'arco might increase the risk of bleeding when taken with anticoagulant or antiplatelet drugs.
In vitro research shows that pau d'arco reduces platelet aggregation and may interfere with vitamin K. One clinical study shows that taking the lapachol constituent of pau d'arco in doses above 1.5 grams daily increases the risk of bleeding. The effects of whole pau d'arco or pau d'arco extract in humans are unclear.
