Suggested use: As an herbal supplement, take one to two capsules daily as needed, preferably at bedtime, for relief of occasional constipation or as directed by your healthcare professional. Anthraquinone containing substances usually produce a soft to semi-fluid stool 6-12 hours after ingestion. It is best to start with one capsule and increase to two, if needed, to get results. Not for long-term use in excess of eight days. Do not exceed recommended dose.

The biggest concern is with warfarin (Coumadin), a blood thinner. Cascara sagrada can trigger diarrhea, which speeds up how warfarin works in your body, raising your bleeding risk.
The same diarrhea effect can also be risky if you take digoxin (Lanoxin) for heart rhythm—cascara can lower your potassium levels, and low potassium makes digoxin toxicity more likely.
If you take water pills (diuretics) or corticosteroids like prednisone, cascara sagrada adds to their potassium-lowering effect, especially with long-term use. Don't combine it with other stimulant laxatives, as they'll compound fluid and electrolyte losses together.
Cascara may also weakly speed up how your liver processes certain medications that depend on CYP3A4 enzymes.

Cytochrome P450 3A4 (Cyp3A4) Substrates
Theoretically, cascara sagrada might decrease the effects of CYP3A4 substrates.
In vitro research suggests that cascara sagrada can induce CYP3A4 enzymes, albeit to a much lower degree than rifampin, a known CYP3A4 inducer.
