Peony is generally well tolerated for short-term use when taken by mouth, though high-quality safety data are limited. The most common side effects from oral use are gastrointestinal — abdominal bloating, loss of appetite, diarrhea, nausea, and general stomach discomfort.

In one clinical trial, diarrhea occurred in 5% of people taking a peony glucoside extract at a high dose for 24 weeks, versus 1% on placebo. There's also one case report of bleeding (easy gum bleeding, nosebleeds, and bruising) in a person on warfarin who switched to a different Chinese herbal product containing peony.
