Moderate evidence
The evidence for Cissus quadrangularis is mixed and modest. It's rated possibly effective for obesity, based on small clinical studies suggesting it may help reduce fasting blood sugar in people who are overweight or obese.
For other uses — high cholesterol (hyperlipidemia), weak bones (osteopenia), cancer, and epilepsy — the evidence we have isn't strong enough to say whether it works or doesn't.

Well-documented data
Cissus quadrangularis is generally well tolerated in the short term. The most common complaints in trials were headache, insomnia, flatulence, diarrhea, indigestion (gastritis), and dry mouth — but these occurred at about the same rate as placebo, meaning they're not clearly caused by the supplement itself.
The big caveat: long-term safety hasn't been well studied, so we don't know what happens if you take it for months or years. During pregnancy or breastfeeding, the safety data isn't there — there isn't enough information to know if it's safe or not.
