HelloPharmacist Scorecard of Cubeb by Hawaii Pharm
Our pharmacy team’s full take, with four database checks built into the cards below — a summary of what is known, not a grade of the product itself.
What’s inside
Full disclosure
This product contains one active ingredient: cubeb fruit extract (in dry form). Cubebs are the dried berries of a pepper plant, traditionally used in cooking and herbal practice.
The liquid also includes alcohol, water, and glycerin as inactive ingredients to deliver the extract.
Does it work?
Couldn't assess
We don't hold effectiveness ratings for cubeb in our data, so the evidence for what it may do isn't established in our monograph.
How safe is it?
Some data gaps
Cubeb is likely fine in small culinary amounts, but safety at medicinal doses — and especially the essential oil — isn't well studied. The safety data advises against taking medicinal amounts during pregnancy and while breastfeeding, since there isn't enough reliable information to know it's safe in those situations.
If you're pregnant or nursing and thinking about using this product, talk with your doctor or pharmacist for personalized advice.
Meds to double-check
Moderate interaction found
Before taking cubeb, check with your doctor or pharmacist if you take any drugs broken down by the liver enzyme CYP3A4 (such as lovastatin, clarithromycin, cyclosporine, or diltiazem). Also double-check if you use H2-blockers (cimetidine, ranitidine, nizatidine, famotidine), proton pump inhibitors like omeprazole or esomeprazole, or antacids.
The bottom line
Scorecard at a glanceFully disclosed formula with no assessable stated purpose. Moderate medication interactions have been identified, and some safety-information gaps remain.
Cubeb is a traditional culinary ingredient, but as a medicinal supplement, its safety and effectiveness aren't well established. If you take medications metabolized by your liver, or medications to control stomach acid, you'll want to check with your doctor or pharmacist before adding this — and definitely skip it if you're pregnant or breastfeeding.

Cubeb by Hawaii Pharm Drug Interactions
HelloPharmacist Interaction Report
Cubeb by Hawaii Pharm may interact with medications, though the evidence comes mostly from lab studies rather than real-world use.
The main concern is with drugs broken down by a liver enzyme called CYP3A4 (cytochrome P450 3A4). In test tubes, compounds in cubebs can slow this enzyme's work, which theoretically could raise the levels and side effects of drugs like lovastatin (a cholesterol medication), clarithromycin (an antibiotic), cyclosporine (an immunosuppressant), and diltiazem (a heart medication) — but this effect hasn't actually been seen in people yet, so the severity is moderate and uncertain.
Read the full breakdown — every affected drug type, severity by severity
Cubebs is also thought to increase stomach acid. Because of that, it might make H2-blockers (such as cimetidine, ranitidine, nizatidine, and famotidine) and proton pump inhibitors or PPIs (omeprazole, lansoprazole, rabeprazole, pantoprazole, and esomeprazole) less effective at reducing acid — the same goes for antacids.
These are minor interactions in theory, not yet confirmed in humans.
Altogether, these interactions span 666 individual medications. Before you start cubeb, run a list of your current medications through the checker on this page, and talk it over with your doctor or pharmacist.

Cubeb Fruit Extract, Dry
Cytochrome P450 3A4 (Cyp3A4) Substrates
Several compounds isolated from cubebs can inhibit cytochrome P450 3A4 (CYP3A4) in vitro. So far, this effect has not been reported in humans. Theoretically, cubebs might increase the effects and side effects of drugs metabolized by CYP3A4. These include lovastatin (Mevacor), clarithromycin (Biaxin), cyclosporine (Neoral, Sandimmune), diltiazem (Cardizem), estrogens, indinavir (Crixivan), triazolam (Halcion), and numerous others.
Antacids
Theoretically, due to reports that cubebs increases stomach acid, cubebs might decrease the effectiveness of antacids.
H2-Blockers
Theoretically, due to reports that cubebs increases stomach acid, cubebs might decrease the effectiveness of H2-blockers. The H2 blockers include cimetidine (Tagamet), ranitidine (Zantac), nizatidine (Axid), and famotidine (Pepcid).
Proton Pump Inhibitors (Ppis)
Theoretically, due to reports that cubebs increases stomach acid, cubebs might decrease the effectiveness of PPIs. PPIs include omeprazole (Prilosec), lansoprazole (Prevacid), rabeprazole (Aciphex), pantoprazole (Protonix), and esomeprazole (Nexium).
