Why this applies to Acalabrutinib

Acalabrutinib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Cubebs interacts with.

Several compounds isolated from cubebs can inhibit cytochrome P450 3A4 (CYP3A4) in vitro. So far, this effect has not been reported in humans. Theoretically, cubebs might increase the effects and side effects of drugs metabolized by CYP3A4. These include lovastatin (Mevacor), clarithromycin (Biaxin), cyclosporine (Neoral, Sandimmune), diltiazem (Cardizem), estrogens, indinavir (Crixivan), triazolam (Halcion), and numerous others.

Here's the gist for your cancer medicine. Acalabrutinib (Calquence) is cleared from your body mainly by a liver enzyme called CYP3A4. In lab studies, certain compounds in cubeb berries can slow that same enzyme down. If that happened in real life, your body would clear acalabrutinib a little slower, which could let more of it build up and possibly raise the chance of side effects like bruising or bleeding, headache, diarrhea, infections, or feeling unusually tired.

I want to be honest about how shaky this is. So far this has only been seen in a test tube, never proven in people, so I wouldn't lose sleep over a sprinkle of cubeb as a spice. Concentrated cubeb extracts or oils are the real question mark. Since acalabrutinib is a serious cancer drug, please run any cubeb supplement past your oncology team first.

What Is Acalabrutinib Used For?

Brand name: Calquence

Calquence is used to treat adults with mantle cell lymphoma (MCL) — either newly diagnosed (in combination with bendamustine and rituximab, for patients who are not candidates for a stem cell transplant) or previously treated (after at least one prior therapy). It is also used to treat adults with chronic lymphocytic leukemia (CLL) or small lymphocytic lymphoma (SLL).

Both the capsule and tablet forms of Calquence are approved for all three of these uses. The specific combination regimen and treatment schedule depend on the condition being treated and what other medicines are used alongside it — your care team will map that out for you.
