All other uses listed — gentian for wound healing, obesity, and gastritis; hops for cognitive decline, sexual dysfunction, anxiety, ADHD, indigestion, and insomnia; horehound and blessed thistle for digestion; and elder and linden for colds, cough, asthma, headache, and other conditions — lack sufficient reliable evidence to establish that they work.

How safe is it?
Well-documented data
Gentian, fenugreek, horehound, blessed thistle, elder, and linden all advise against use in pregnancy due to insufficient safety data or traditional concerns about effects on the uterus. Fenugreek is likely unsafe in pregnancy; the others are best avoided.
For breastfeeding, fenugreek is possibly safe (often used to boost milk supply, though evidence is limited), blessed thistle and linden are used traditionally for milk supply but safety is not well studied, and gentian, hops, and elder are best avoided due to lack of safety information. Common side effects include stomach upset (gentian, fenugreek, horehound, blessed thistle), nausea and vomiting (fenugreek, elder), diarrhea (fenugreek), dizziness (horehound, elder), and drowsiness (hops).
Fenugreek can cause allergic reactions including bronchospasm and swelling, and may cause an unusual body odor in newborns if taken near delivery. Blessed thistle in high doses (over 5 grams per cup of tea) can cause stomach irritation and vomiting.
Elder is toxic if raw or unripe.

Fenugreek specifically affects diabetes medications (it can lower blood sugar), the heart drug theophylline, the blood pressure medicine metoprolol, the seizure drug phenytoin (Dilantin), and the erectile dysfunction drug sildenafil (Viagra), potentially reducing how well each one works.
Hops may add to the drowsiness of sedative drugs and might interfere with hormone replacement therapy. Horehound may lower blood sugar along with diabetes drugs.
Elder flower may raise levels of a broad class of medications your liver metabolizes (CYP3A4 substrates), and both elder flower and linden could increase lithium levels in your blood if you take it for mood. Hops strobile extract
Cns Depressants
Theoretically, concomitant use of hops with sedative drugs might cause additive sedation.
Some animal research shows that hops has sedative effects.
Estrogens
Theoretically, concomitant use of large amounts of hops might interfere with hormone replacement therapy due to competition for estrogen receptors.
In vitro research suggests that certain hops constituents can competitively bind to estrogen receptors. However, most hops extracts contain very small amounts of these constituents.
Cytochrome P450 1A2 (Cyp1A2) Substrates
Hops extract does not seem to affect the metabolism of CYP1A2 substrates.
In vitro research suggests that flavonoid constituents of hops inhibit CYP1A2 enzyme activity. However, a pharmacokinetic study in healthy postmenopausal patients shows that taking a standardized extract of spent hops containing prenylated phenols, as 59.5 mg twice daily for 2 weeks, does not affect levels of caffeine, a CYP1A2 probe substrate.
Cytochrome P450 3A4 (Cyp3A4) Substrates
Theoretically, hops extract might alter metabolism of CYP3A4 substrates; however, this effect may not be clinically significant.
Animal research suggests that specific constituents of hops, called lupulones, can induce hepatic CYP3A4 enzyme activity. However, a pharmacokinetic study in healthy postmenopausal patients with normal metabolism shows that taking a standardized extract of spent hops containing prenylated phenols, as 59.5 mg twice daily for 2 weeks, decreases the concentration of alprazolam, a CYP3A4 probe substrate, by 7.6%. This reduction is unlikely to be clinically relevant.
Elder flower extract
Cytochrome P450 3A4 (Cyp3A4) Substrates
Theoretically, taking American elder might increase levels of CYP3A4 substrates.
In vitro research shows that American elder inhibits CYP3A4 enzymes.
Lithium
Theoretically, American elder might reduce excretion and increase levels of lithium.
American elder is thought to have diuretic properties. Theoretically, American elder might reduce the excretion of lithium, increasing serum levels.
Fenugreek Seed Extract
Anticoagulant/Antiplatelet Drugs
Theoretically, fenugreek might have additive effects when used with anticoagulant or antiplatelet drugs.
Some of the constituents in fenugreek have antiplatelet effects in animal and in vitro research. However, common fenugreek products might not contain sufficient concentrations of these constituents for clinical effects. A clinical study in patients with coronary artery disease or diabetes shows that taking fenugreek seed powder 2.5 grams twice daily for 3 months does not affect platelet aggregation, fibrinolytic activity, or fibrinogen levels .
Antidiabetes Drugs
Theoretically, fenugreek seed might have additive hypoglycemic effects when used with antidiabetes drugs.
Clinical research shows that fenugreek seed can reduce fasting blood glucose and 2-hour postprandial glucose levels in adults with type 2 diabetes.
Clopidogrel (Plavix)
Theoretically, fenugreek seed might alter the clinical effects of clopidogrel by inhibiting its conversion to the active form.
Animal research shows that fenugreek seed 200 mg/kg daily for 14 days increases the maximum serum concentration of clopidogrel by 21%. It is unclear how this affects the pharmacokinetics of the active metabolite of clopidogrel; however, this study found that concomitant use of fenugreek seed and clopidogrel prolonged bleeding time by an additional 11%.
Metoprolol (Toprol)
Theoretically, fenugreek seed might have additive hypotensive effects when used with metoprolol.
Animal research shows that fenugreek seed 300 mg/kg daily for 2 weeks decreases systolic and diastolic blood pressure by 9% and 11%, respectively, when administered alone, and by 15% and 22%, respectively, when given with metoprolol 10 mg/kg.
Phenytoin (Dilantin)
Theoretically, fenugreek might decrease plasma levels of phenytoin.
Animal research shows that taking fenugreek seeds for 1 week decreases maximum concentrations and the area under the curve of a single dose of phenytoin by 44% and 72%, respectively. This seems to be related to increased clearance. So far, this interaction has not been reported in humans.
Sildenafil (Viagra)
Theoretically, concurrent use of sildenafil and fenugreek might reduce levels and therapeutic effects of sildenafil.
Animal research shows that taking fenugreek seeds for 1 week reduces maximum concentrations and the area under the curve of a single dose of sildenafil by 27% and 48%, respectively. So far, this interaction has not been reported in humans.
Theophylline
Theoretically, fenugreek may reduce the levels and clinical effects of theophylline.
Animal research shows that fenugreek 50 grams daily for 7 days reduces the maximum serum concentration (Cmax) of theophylline by 28% and the area under the plasma drug concentration-time curve (AUC) by 22%.
Warfarin (Coumadin)
Theoretically, fenugreek might have additive effects with warfarin and increase the international normalized ratio (INR).
Some fenugreek constituents have antiplatelet effects, although these might not be present in concentrations that are clinically significant. In one case report, a patient taking warfarin experienced an increased INR when starting to take fenugreek in combination with boldo.
Antihypertensive Drugs
Fenugreek may also have an additive effect on blood pressure-lowering medications. Studies on animals have shown that fenugreek seed can decrease both systolic and diastolic blood pressure by up to 22% when combined with metoprolol. Therefore, it is essential to monitor your blood pressure regularly if you are taking fenugreek and metoprolol together or any other antihypertensive drugs.
Horehound flowering tops extract
Antidiabetes Drugs
Evidence from animal research suggests that white horehound might lower blood glucose. However, preliminary clinical research suggests that white horehound only slightly lowers blood glucose when taken in combination with antidiabetic agents, suggesting that the effect may not be clinically significant. Until more is known, use with caution. Theoretically, white horehound may have additive effects when used by patients taking antidiabetic drugs. The dose of diabetes medications might need to be adjusted. Some antidiabetes drugs include glimepiride (Amaryl), glyburide (DiaBeta, Glynase PresTab, Micronase), insulin, metformin (Glucophage), pioglitazone (Actos), rosiglitazone (Avandia), and others.
Antihypertensive Drugs
Evidence from animal research suggests that taking white horehound extract lowers blood pressure. Theoretically, taking white horehound with antihypertensive drugs might enhance therapeutic effects and increase the risk of hypotension. Some antihypertensive drugs include captopril (Capoten), enalapril (Vasotec), losartan (Cozaar), valsartan (Diovan), diltiazem (Cardizem), Amlodipine (Norvasc), hydrochlorothiazide (HydroDiuril), furosemide (Lasix), and many others.
Gentian root extract
Antihypertensive Drugs
Theoretically, taking gentian with antihypertensive drugs might increase the risk of hypotension.
In vitro research shows that gentian can cause vasodilation and lower blood pressure.
Blessed Thistle Herb Extract
Antacids
Theoretically, blessed thistle might decrease the effectiveness of antacids.
There are reports that blessed thistle increases stomach acid.
H2-Blockers
Theoretically, blessed thistle might decrease the effectiveness of H2-blockers.
There are reports that blessed thistle increases stomach acid.
Proton Pump Inhibitors (Ppis)
Theoretically, blessed thistle might decrease the effectiveness of PPIs.
There are reports that blessed thistle increases stomach acid.
Linden Flower, Leaf Extract
Lithium
Theoretically, due to its potential diuretic effects, linden might increase the effects and adverse effects of lithium.
Linden is thought to have diuretic properties. The dose of lithium might need to be decreased.
