Acetaminophen, Butalbital, Caffeine, Codeine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group American Elder interacts with.

Theoretically, taking American elder might increase levels of CYP3A4 substrates.

This combination pill has a few moving parts, and the piece that touches American elder is the liver enzyme called CYP3A4. Lab studies (test tube only, not people) hint that elder may slow this enzyme down. If that happened in real life, it could let some of these ingredients build up a bit. For the butalbital portion especially, higher levels could mean more drowsiness, dizziness, or feeling foggy. Codeine is a little different since it relies on a separate enzyme (CYP2D6) to turn into its active form, so the elder effect there is less clear.

Honestly, the evidence is weak and theoretical. A bit of cooked elderberry in a cold remedy is unlikely to cause trouble, but concentrated extracts are less predictable. Run it by your pharmacist or doctor to be safe.
