Modafinil falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group American Elder interacts with.

Theoretically, taking American elder might increase levels of CYP3A4 substrates.

Here's the gist with modafinil: your body partly clears it using a liver enzyme called CYP3A4. Lab (test-tube) studies hint that American elder may slow that enzyme down. If that happened in real life, modafinil could clear a little more slowly and build up, which might mean stronger effects like jitteriness, headache, a racing or pounding heart, trouble sleeping, or feeling anxious.

I want to be straight with you, though: this is only a theoretical concern based on lab data, not on people taking these two together. Real-world impact is uncertain, and culinary amounts of properly prepared elderberry are far less likely to matter than concentrated extracts. Still, it's worth a quick chat with your pharmacist or doctor before adding an elder supplement.

American elder is a North American plant whose cooked, ripe berries and flowers are used mainly for colds, flu, and general immune support. The science behind these uses is limited and most studies focus on the closely related European black elder (Sambucus nigra), so benefits are uncertain. Only properly prepared berries and flowers should be used, since raw berries, leaves, bark, and seeds can cause poisoning.

What Is Modafinil Used For?

Brand names: Provigil, Sparlon, Alertec

Modafinil (also sold as Provigil) is used to improve wakefulness in adults who experience excessive sleepiness due to narcolepsy, obstructive sleep apnea (OSA), or shift work disorder (SWD).

In OSA, modafinil treats the sleepiness — not the underlying airway obstruction itself. It is used alongside standard treatments such as CPAP (a breathing device worn during sleep), not as a replacement for them. If CPAP is appropriate for you, your doctor will want you to use it consistently even while taking modafinil.
