Papain is generally well tolerated when used short-term and at normal doses, though some people have allergic reactions — itchy eyes, runny nose, sneezing, stomach cramps, or diarrhea. High doses may cause serious problems like esophageal perforation or severe inflammation of the stomach lining.

Pepsin and betaine HCl are generally well tolerated too, but betaine HCl can irritate the stomach and should be avoided if you have ulcers, gastritis, or reflux. During pregnancy, concentrated papain and unripe papaya products aren't recommended due to safety concerns, and there isn't enough data to know if supplemental papain is safe while breastfeeding — talk with your doctor or pharmacist before using it.
