Alfalfa leaf is generally well tolerated in food amounts, but supplement doses carry some risks. The most common side effects are digestive: abdominal discomfort, diarrhea, gas, and loose stools—these showed up in case studies, particularly in the first week of use.

One rare but serious reported effect is a lupus-like syndrome from long-term use. Skin reactions (dermatitis) have also been documented after weeks of regular consumption.

Alfalfa contains a large amount of vitamin K. This could theoretically interfere with the activity of warfarin.
