Gotu kola is generally well tolerated in the short term, but quality evidence for long-term safety is limited and liver concerns have been reported. The most common side effects when taken by mouth are gastric irritation and nausea.

Rare but serious liver damage (hepatotoxicity) has been reported in at least four cases, though whether gotu kola was the direct cause remains unclear — contaminants or undisclosed doses may have played a role. When applied to skin, it can occasionally trigger eczema or allergic contact dermatitis with redness, itching, and burning.
