Here is the interaction we found between these two, with the evidence behind it.
HEPATOTOXIC DRUGS
Why this applies to Acetaminophen, Butalbital, Caffeine
Acetaminophen, Butalbital, Caffeine falls under HEPATOTOXIC DRUGS — the group Gotu Kola interacts with.
There are at least four case reports of hepatotoxicity associated with the use of gotu kola. However, more information is needed to determine if gotu kola was the causative factor in these cases.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the simple version: this is a liver concern, not a change in how your headache medicine works. The acetaminophen part of your Esgic or Fioricet is already processed by the liver, and it's the one ingredient that can stress the liver if you take too much or use it daily. Gotu kola has been tied to a small number of liver injury reports, so the worry is that the two together could add up and be harder on your liver.
I'll be honest, the evidence here is thin, just a few anecdotal cases, so this isn't a reason to panic. Still, watch for things like yellowing skin or eyes, dark urine, belly pain, or unusual tiredness. Please check with your pharmacist or doctor before combining them, especially if you drink alcohol or already have liver issues.

From our pharmacist’s overview on the Acetaminophen, Butalbital, Caffeine page
“Alcohol is the interaction I would flag first for anyone taking acetaminophen, butalbital, and caffeine (Esgic, Fiogesic, and others). Drinking raises the risk of liver injury from the acetaminophen component, and 38 of the 252 supplement ingredients on file carry that same liver concern.”
