This combination pill includes codeine, an opioid that can already make you drowsy. The concern with gotu kola is mostly about that calming, sleepy side. Lab studies hint that gotu kola may gently act on GABA receptors in the brain, which are the same calming pathways opioids lean on. Stacked together, you might feel more tired, foggy, dizzy, or slowed down than the medicine alone would cause.

I want to be honest: this is a theoretical worry based on test-tube research, not real patient reports, so it may not amount to much. Still, until you know how the pair affects you, avoid driving and skip alcohol. If you feel unusually groggy, check in with your pharmacist or doctor before changing anything.

There are at least four case reports of hepatotoxicity associated with the use of gotu kola. However, more information is needed to determine if gotu kola was the causative factor in these cases.

Here's the thing to keep in mind with this combo pill. One of its ingredients, acetaminophen (the same drug in Tylenol), is processed by your liver and is well known for being hard on the liver in high doses. Gotu kola has been linked, in a handful of case reports, to liver irritation too. So the worry isn't that one changes how the other works. It's that both may stress the liver, and those effects could potentially add up.

I want to be honest about the strength of this: it's theoretical and based only on a few anecdotal cases, so it's far from proven. Still, watch for yellowing of the skin or eyes, dark urine, belly pain, or unusual tiredness. If you take this medication regularly or enjoy alcohol, talk with your pharmacist or doctor before adding gotu kola.
