Regular use of graviola raises some safety concerns. The most common side effects from oral use are stomach pain and nausea, though these are generally mild.

More rarely, long-term use has been linked to movement disorders and nerve damage (myeloneuropathy) that can resemble Parkinson disease symptoms. Lab studies also suggest possible nerve toxicity with extended use.

Additionally, even low amounts of graviola fruit or tea may worsen motor and cognitive symptoms in people who already have Parkinson disease. Graviola is not recommended during pregnancy or while breastfeeding because there isn't enough safety data.
