What’s inside
Full disclosure
This product contains one active ingredient: L-Carnitine at 500 mg per capsule. L-Carnitine is an amino acid your body uses to move fats into cells so they can be burned for energy — your kidneys and liver make it naturally, and you also get it from meat and dairy.
The inactive ingredients are cellulose, stearate, and silica.
Does it work?
Moderate evidence
L-Carnitine is rated effective for treating L-carnitine deficiency. For other uses — high cholesterol (hyperlipidemia), heart failure (congestive heart failure), chest pain (angina) — the evidence is rated possibly effective, meaning there's some research support but it's not conclusive yet.
How safe is it?
Well-documented data
L-Carnitine is generally well tolerated at typical doses. The most common side effects are stomach upset — abdominal cramps, pain, diarrhea, nausea, and heartburn — and a fish-like body odor, especially at higher doses.
Seizures are rare but have been reported. The safety data for use while breastfeeding is not well studied, so caution is advised — talk with your doctor or pharmacist if you're nursing.
Pregnancy safety data isn't on file for this ingredient.

Moderate interaction found
Check your medications against this supplement if you take blood thinners like warfarin or acenocoumarol (Moderate severity) or thyroid hormone replacement (Moderate severity). Both interaction types need a pharmacist review before you begin.

L-Carnitine 500 mg by Life Extension Drug Interactions
HelloPharmacist Interaction Report
Life Extension's L-Carnitine 500 mg interacts with blood thinners (anticoagulants) and thyroid hormone replacement — the two main medication types to watch before you start.
Read the full breakdown — every affected drug type, severity by severity
The most serious concern is blood thinners. L-Carnitine may increase how strongly warfarin (Coumadin) works, raising your bleeding risk.
The same goes for acenocoumarol (Sintrom), a shorter-acting blood thinner — there are case reports of people's clotting levels (INR) rising when they combined it with L-Carnitine. Both interactions are rated Moderate severity.
If you take thyroid hormone replacement (for an underactive thyroid), L-Carnitine might make it less effective by blocking the hormone from entering your cells properly. This is also Moderate severity.
Altogether, these interactions span 19 individual medications.
Use the medication checker on this page to look up your exact drugs before you add this supplement.

For each ingredient in L-Carnitine 500 mg with known interactions, here are the types of medications it can affect. Open any type for the detail — or search your exact drug in the checker above.
L-Carnitine
Acenocoumarol (Sintrom)
Theoretically, L-carnitine might increase the anticoagulant effects of acenocoumarol.
L-carnitine might enhance the anticoagulant effects of acenocoumarol, an oral anticoagulant similar to warfarin, but shorter-acting. There are at least two case reports of INR elevation with concomitant use. In one case, a 33-year-old male with a previously stable INR had an elevated INR of 4.65 after L-carnitine was started and continued for 10 weeks. INR normalized after discontinuation of the L-carnitine-containing product.
Thyroid Hormone
Theoretically, L-carnitine might decrease the effectiveness of thyroid hormone replacement.
L-carnitine appears to act as a peripheral thyroid hormone antagonist by inhibiting entry of thyroid hormone into the nucleus of cells. Taking L-carnitine also seems to diminish some of the symptoms of hyperthyroidism.
Warfarin (Coumadin)
Theoretically, L-carnitine might increase the anticoagulant effects of warfarin.
L-carnitine might increase the anticoagulant effects of acenocoumarol, a shorter-acting oral anticoagulant similar to warfarin. There is not enough information to know whether this interaction occurs with L-carnitine and warfarin.
