What’s inside
Full disclosure
This product contains one active ingredient: L-carnitine, an amino acid that plays a role in how your cells produce energy. It's an extra-strength capsule formula.
The inactive ingredients are cellulose and magnesium stearate, which are common capsule fillers and binders.
Does it work?
Strong evidence
L-carnitine is effective for treating L-carnitine deficiency. For other uses, the evidence is mixed: it's possibly effective for high cholesterol (hyperlipidemia), congestive heart failure, and angina.
That means there's some research support, but the data isn't conclusive enough to call it fully proven. Talk with your doctor or pharmacist about whether this supplement makes sense for your specific situation.
How safe is it?
Well-documented data
L-carnitine is generally well tolerated at typical doses. The most common side effects are stomach-related: abdominal cramps or pain, diarrhea, nausea, heartburn, and loss of appetite.
A fishy body odor has also been reported. High doses can make these worse.
Seizures are rare but serious. There isn't enough reliable safety data for pregnant people, so medical supervision is advised.
Safety while breastfeeding hasn't been well studied, so caution is advised if you're nursing — talk with your doctor or pharmacist.
Meds to double-check
Moderate interaction found
Before you start, double-check if you take warfarin or acenocoumarol (blood thinners) — L-carnitine may boost their effects and raise your bleeding risk. Also check if you take thyroid hormone replacement; L-carnitine might make it less effective.
Use the medication checker on this page to verify your exact drugs.
The bottom line
Scorecard at a glanceFully disclosed formula with clinical evidence supporting its stated purpose. Moderate medication interactions have been identified, and safety information is well characterized.
L-carnitine can help if you have a documented deficiency or want to explore it for heart health or cholesterol — but it's not for everyone. If you take blood thinners or thyroid hormone replacement, you need to check with your pharmacist first; these interactions could affect how well your medications work.
Stomach upset is common, so start low and see how you feel.

L-Carnitine Extra Strength by BRI Nutrition Drug Interactions
HelloPharmacist Interaction Report
L-Carnitine Extra Strength by BRI Nutrition interacts with blood thinners (anticoagulants) and thyroid hormone replacement.
The most serious concern is with warfarin (Coumadin) and acenocoumarol (Sintrom), both of which may have their blood-thinning effects increased by L-carnitine — meaning your bleeding risk could go up. Acenocoumarol has case reports of this happening; warfarin's risk is theoretical but still important to flag.
Read the full breakdown — every affected drug type, severity by severity
Thyroid hormone replacement therapy (for low thyroid) may become less effective when you take L-carnitine, since the supplement appears to block how your cells take up thyroid hormone. This could mean your thyroid symptoms return or worsen.
Although no interactions are documented for other medication types in our data, check your exact medications with the search tool on this page — especially if you take any blood thinner or thyroid medication.

For each ingredient in L-Carnitine Extra Strength with known interactions, here are the types of medications it can affect. Open any type for the detail — or search your exact drug in the checker above.
L-Carnitine
Acenocoumarol (Sintrom)
Theoretically, L-carnitine might increase the anticoagulant effects of acenocoumarol.
L-carnitine might enhance the anticoagulant effects of acenocoumarol, an oral anticoagulant similar to warfarin, but shorter-acting. There are at least two case reports of INR elevation with concomitant use. In one case, a 33-year-old male with a previously stable INR had an elevated INR of 4.65 after L-carnitine was started and continued for 10 weeks. INR normalized after discontinuation of the L-carnitine-containing product.
Thyroid Hormone
Theoretically, L-carnitine might decrease the effectiveness of thyroid hormone replacement.
L-carnitine appears to act as a peripheral thyroid hormone antagonist by inhibiting entry of thyroid hormone into the nucleus of cells. Taking L-carnitine also seems to diminish some of the symptoms of hyperthyroidism.
Warfarin (Coumadin)
Theoretically, L-carnitine might increase the anticoagulant effects of warfarin.
L-carnitine might increase the anticoagulant effects of acenocoumarol, a shorter-acting oral anticoagulant similar to warfarin. There is not enough information to know whether this interaction occurs with L-carnitine and warfarin.
