Does it work?
Moderate evidence
Lemon balm is possibly effective for cold sores (herpes labialis) and for stress relief. There's also possibly effective evidence for depression.
Keep in mind that 'possibly effective' means the evidence exists but isn't yet definitive — more research would help confirm how well it works for these uses.
How safe is it?
Well-documented data
Lemon balm is generally well tolerated for short-term use in most healthy adults, though long-term safety and high-dose effects haven't been well studied. Dizziness and sedation have been reported orally, but they don't seem to happen more often than with placebo.
Topically, it's been linked to rare cases of skin irritation and cold sore worsening, again no more often than placebo. One case report described a heart rhythm problem (atrioventricular block) in a woman using a multi-ingredient product containing lemon balm, but it's unclear whether lemon balm alone was responsible — symptoms reversed when she stopped the product.
During pregnancy, there isn't enough reliable safety data, so it's best avoided unless your doctor advises otherwise. While breastfeeding, safety information is limited; talk with your doctor or pharmacist before use.

Theoretically, concomitant use of lemon balm might have additive effects with CNS depressant drugs.
Lemon balm seems to have CNS depressant activity in animals and in humans.
Thyroid Hormone
Theoretically, lemon balm might interfere with thyroid hormone replacement therapy.
In vitro, constituents of lemon balm extract bind to thyroid stimulating hormone (TSH), preventing TSH receptor-binding and leading to the inhibition of TSH-stimulated adenylate cyclase activity. In animals, lemon balm extract has been shown to decrease levels of circulating TSH and inhibit thyroid secretion.
