Cordyceps is generally well tolerated short-term (up to about a year), though the quality of mushroom supplements varies widely and long-term safety isn't well studied. The most common side effects are abdominal discomfort, constipation, and diarrhea — usually mild and sometimes reduced by taking it with food.

Lion's mane and maitake are also generally well tolerated, with occasional gastrointestinal discomfort and nausea reported. Shiitake is fine as a food but less data exist for concentrated supplements; raw shiitake can cause a distinctive skin rash (shiitake dermatitis), and handling it can trigger allergic reactions in sensitive people.

Chaga has caused kidney problems in rare cases due to its high oxalate content, and most of these ingredients lack safety data in pregnancy and breastfeeding — the facts advise against use in both situations. The elevated INR resolved after holding warfarin for two days, then reducing the dose by 11%. It is thought that the beta-glucan constituent of maitake mushroom might cause warfarin dissociation from proteins, resulting in increased free warfarin levels and increased warfarin effects.
