Tribulus is generally well tolerated in the short term for healthy adults, but long-term safety data and quality control information are limited. Serious side effects are rare but have been reported — including liver and kidney injury, seizures, and in one extreme case, lower limb weakness and kidney failure after consuming very large amounts.

More commonly, it may cause abdominal pain, cramping, nausea, vomiting, diarrhea, or constipation, though one study found these complaints occurred at similar rates in people taking tribulus and those on placebo.
