Phosphatidylserine is possibly effective for age-related cognitive decline and Alzheimer disease, according to the evidence we have on file. For athletic performance, ADHD, general cognitive function, and dementia, the evidence isn't strong enough to rate it one way or the other — we don't hold enough reliable data to know whether it works for those uses.
How safe is it?
Well-documented data
Phosphatidylserine is generally well tolerated in studies, though long-term safety hasn't been fully established. The most common side effects are flatulence, stomach upset, headache, trouble sleeping, and nausea.
Insomnia is more likely at higher doses around 600 mg, and stomach upset can happen at doses of 200–300 mg per day. There isn't enough safety information to use phosphatidylserine during pregnancy, so it's best to avoid it then.
Safety while breastfeeding is also unknown, so avoidance is recommended. Talk with your doctor or pharmacist about your individual situation if you're pregnant, planning to become pregnant, or breastfeeding.
