How safe is it?
Well-documented data
American ginseng is generally well tolerated short-term in healthy adults, though quality and dosing vary and long-term safety hasn't been well studied. The main reported side effect is headache.
The product should be avoided during pregnancy (not enough safety data) and breastfeeding. Gotu kola is generally considered well tolerated short-term, but there's a concern about liver toxicity—at least four case reports exist, though causation isn't confirmed.
Common side effects are gastric irritation and nausea. It's traditionally avoided in pregnancy and should be avoided while breastfeeding.
Lemon balm is generally well tolerated for short-term use, though high-dose and long-term safety aren't well studied. Dizziness and sedation have been reported, though they don't appear more common than placebo.
It should be avoided in pregnancy unless your doctor advises otherwise, and caution is needed while breastfeeding.

American ginseng may also interfere with immunosuppressants (drugs that quiet your immune system), increase the risk of low blood sugar (hypoglycemia) when combined with diabetes medications, and theoretically interfere with MAOIs (a class of older antidepressants).
Gotu kola and lemon balm can both add to the drowsy effects of CNS depressants (sedatives, sleep aids, and similar drugs). Gotu kola may also increase the risk of liver injury if you're taking other hepatotoxic drugs.

Gotu Kola Aerial Extract
Cns Depressants
Theoretically, taking gotu kola might increase the sedative effects of CNS depressants.
In vitro research suggests that gotu kola may have sedative effects via binding of GABA receptors.
Hepatotoxic Drugs
Theoretically, taking gotu kola with hepatotoxic drugs might have additive adverse effects.
There are at least four case reports of hepatotoxicity associated with the use of gotu kola. However, more information is needed to determine if gotu kola was the causative factor in these cases.
Lemon Balm Aerial Extract
Cns Depressants
Theoretically, concomitant use of lemon balm might have additive effects with CNS depressant drugs.
Lemon balm seems to have CNS depressant activity in animals and in humans.
Thyroid Hormone
Theoretically, lemon balm might interfere with thyroid hormone replacement therapy.
In vitro, constituents of lemon balm extract bind to thyroid stimulating hormone (TSH), preventing TSH receptor-binding and leading to the inhibition of TSH-stimulated adenylate cyclase activity. In animals, lemon balm extract has been shown to decrease levels of circulating TSH and inhibit thyroid secretion.
American Ginseng Root Extract
Warfarin (Coumadin)
American ginseng seems to decrease the effectiveness of warfarin therapy.
Healthy patients receiving warfarin 5 mg daily, who also take American ginseng 1 gram twice daily, seem to have a significantly reduced international normalized ratio (INR).
Antidiabetes Drugs
Theoretically, taking American ginseng with antidiabetes drugs might increase the risk of hypoglycemia.
American ginseng seems to lower postprandial blood glucose. Theoretically, concomitant use with antidiabetes drugs might enhance blood glucose lowering effects and possibly cause hypoglycemia.
Immunosuppressants
Theoretically, American ginseng use might interfere with immunosuppressive therapy.
American ginseng seems to stimulate immune function. Theoretically, American ginseng might decrease the effectiveness of immunosuppressant drugs.
Monoamine Oxidase Inhibitors (Maois)
Theoretically, American ginseng can interfere with MAOI therapy.
There is one case report of insomnia, headache, and tremors when an unspecified ginseng product was used with phenelzine (Nardil), an MAOI. There is also one case report of hypomania when an unspecified ginseng product was used with phenelzine. Theoretically, American ginseng may interfere with MAOI therapy.
