Gugul extract is generally well tolerated but commonly causes digestive upset (belching, bloating, diarrhea, nausea, vomiting), headache, unpleasant taste, and skin reactions ranging from mild itching to rash and swelling. A small number of people have developed serious muscle breakdown (rhabdomyolysis), which is rare.

Skin reactions appear dose-dependent and may occur in 3–15% of users depending on the dose. Policosanol is usually well tolerated, though some people report headache, dizziness, sleepiness, irritability, insomnia, upset stomach, skin rash, and weight loss—but these are uncommon in studies.

Guggul should be avoided during pregnancy because it may stimulate the uterus and safety isn't established, and should also be avoided while breastfeeding due to insufficient safety data. Policosanol should likewise be avoided in pregnancy and breastfeeding because there isn't enough safety information to know either way.

Anticoagulant/Antiplatelet Drugs

Theoretically, taking policosanol with other antiplatelet or anticoagulant drugs might increase the risk of bruising and bleeding.
Some clinical research shows that taking policosanol 10-50 mg daily for 7-15 days can inhibit platelet aggregation in healthy patients. Although, one clinical trial shows that taking policosanol 10 mg twice daily for 2 weeks prior to warfarin dosing does not affect warfarin pharmacokinetics or warfarin response. Furthermore, a study in patients undergoing percutaneous coronary intervention with a drug-eluting stent found that taking policosanol 40 mg plus clopidogrel and aspirin daily for 30 days modestly reduced the risk for minor bleeding events when compared with taking clopidogrel and aspirin alone. Until more is known, use with caution.
